Research Abstract

産業医科大学倫理委員会の承認を経て、研究参加者673名よりインフォームド・コンセントを取得し、採血によるゲノムDNAおよび、ニコチン依存の評価尺度を含むアンケート回答を得た。現在までに、グルタミン酸受容体遺伝子、ニコチン性アセチルコリン受容体遺伝子、オピオイド受容体遺伝子について遺伝子等について遺伝子解析を終え、一部については、ニコチン依存との関連を見出した(投稿準備中)。ニコチン依存の度合いについては、タバコ依存症スクリーニング(The Tobacco Dependence Screener : TDS)および、ファーガストロームのニコチン依存度指数(Fagerstrom Test for Nicotine Dependence : FTND)を用い評価した。ニコチン性アセチルコリン受容体については、α4及びβ2サブユニット受容体遺伝子多型とFTND総得点との間に有意な関連を認めた。また、FTND総得点については、上記2つの遺伝子多型の相互作用の影響を認めた。ニコチン性アセチルコリン受容体は、喫煙後脳内に移行したニコチンが、最初に作用する部位であり、いわば脳内報酬系の入口部分であり、その遺伝子の変異がニコチン依存度と関連していることが、分子レベルでも示唆されたことになる。これは、ニコチン依存症の薬物療法の個別化に繋がる知見であり、将来、ニコチン依存症のテーラーメイド医療に役立つ可能性があると考えられる。今後は、他の候補遺伝子についても、ニコチン依存との関連について、詳細な検討を継続的に行い、臨床に役立つ知見を明らかにしていく予定である。
