Outline of Research at the Start |
本研究課題では、パーキンソン病の早期診断と病型分類、病期進行の客観的なバイオマーカーの確立を目的として、先端的なMRI画像解析技術を用いて、パーキンソン病患者の脳機能・構造解析を行う。具体的には先進的なMRI解析技術である神経突起イメージングおよび自由水イメージングなどの拡散MRI技術を用いて早期パーキンソン病患者の軸索変性および神経炎症を検出し、パーキンソン病の臨床的なサブタイプを群別することが可能か検討を行う。さらに、これらの定量値がパーキンソン病における病期進行の予測因子として使用可能なバイオマーカーであるか検討を行う。

Outline of Annual Research Achievements |
This research applied neurite orientation dispersion and density imaging, free-water imaging, and fixel-based analysis to baseline and follow-up data of subjects with Parkinson's disease (PD). As results of cross-sectional studies, we showed different patterns of brain tissue microstructural changes (i.e., neuronal loss and neuroinflammation) in different clinical subtypes of PD (i.e., with vs. without neurocognitive and psychiatric symptoms, tremor dominant vs. postural instability and gait disorder motor subtypes, with vs. without mild cognitive impairment, and with vs. without glucocerebrosidase gene mutations). Furthermore, in a longitudinal analysis, we demonstrated white matter neural compensation among PD patients with normal cognition one year after diagnosis.
