消化器がん患者由来の生体材料を用いて、病態解析と診断のための新技術を研究した。(1)細胞外分泌小胞の『殻』に存在するがん細胞膜に特異的な抗原を指標にした収集法を開発し、(2)その収集したEVの『中身』のRNAの修飾を直接読む技術に発展させ、膵がんの診断での有用性を示した。これらのがんEV抗原と内部RNAを統合した解析により、バイオマーカー技術の最高精度化をはかりつつ膵がん以外のアンメット・ニーズが高い疾患にまで対象を広げて、病態を踏まえ画期的な創薬に応用できる技術基盤を完成させる計画である。

| Outline of Final Research Achievements |
To pioneer a transformative diagnostic approach for intractable gastrointestinal cancers, we developed a cutting-edge molecular platform targeting trace extracellular vesicles (EVs) circulating in the bloodstream. By integrating our proprietary high-sensitivity EV membrane antigen detection system (ExoScreen) with a comprehensive RNA modification profiling technique (TR), we established “Ex-TR”-a novel technology enabling the simultaneous capture of EV surface proteomic signatures and internal epitranscriptomic landscapes. Through validation with paired human plasma and tumor samples, Ex-TR achieved exceptional precision in detecting gastrointestinal malignancies and unveiled multiple previously unrecognized molecular markers. More than a diagnostic tool, Ex-TR represents a next-generation translational platform that bridges molecular diagnosis with pathophysiological insight, offering powerful potential for future therapeutic innovation.

| Academic Significance and Societal Importance of the Research Achievements |
細胞外小胞(EV)を標的とする革新的診断技術「Ex-TR法」により、血液から高精度にがんを識別する新たな分子診断の枠組みを確立した。本研究は、EV膜抗原検出とRNA修飾解析の統合という新規アプローチを提案し、EVの機能的多様性や病態関連性を分子レベルで可視化した点で、学術的にも極めて先駆的である。社会的には、侵襲性の低い液体生検によるがんの早期診断や予後予測を可能とし、個別化医療や創薬の加速に貢献する技術として、将来の医療実装が強く期待される。
