Outline of Research at the Start

本研究の目的は,医療技能のデジタル化(医デジ化)を加速・推進するロボティック超音波診断・治療基盤システムの構築法を確立することである。ここで,ロボティック超音波診断・治療基盤システムとは呼吸等により能動的に運動する患部を抽出・追従・モニタリングするとともに超音波を集束させてピンポイントに患部へ照射することにより,がん組織や結石の治療を患者の皮膚表面を切開することなく非侵襲かつ低負担に行なおうとするものである。
研究期間内に生体に対して90パーセントの体動を補償し,1mmの精度で生体の患部を抽出・追従・モニタリングするシステムを実現,動物実験レベルでシステムの有効性および安全性を実証する。

Outline of Annual Research Achievements

本研究課題の目的は、人工知能技術・ロボット技術・超音波技術を用いてロボティック超音波診断・治療基盤システムの構築法を確立することであり、下記の5つのコア基盤技術に関する研究を遂行し、下記に示すようにきわめて順調に優れた成果を積み重ねてきている。(コア技術I) 機能に応じた機構設計技術、 (コア技術II) 医療診断・治療技能における機能の抽出・構造化技術、 (コア技術III) 患者に対するロボットの安全・安心動作技術、(コア技術IV) 診断・治療タスクに応じたシステム動作技術、(コア技術V) リアルタイム医用画像処理技術。
とりわけ(コア技術I)に関して、本プロジェクトにより世界で初めて超音波診断・治療ロボットのみならず,診断対象である患部の埋め込まれた患者ロボットのプロトタイプをも統合的に構築したシステムが具現化された。この世界で初めて具現化された『超音波診断・治療ロボットおよび診断対象である患部の埋め込まれた患者ロボットの統合ロボティック・システム』という、極力あえて人間を介さない統合システム上において今後、デジタルに実装される機能関数群の構造によって構成論的に『医療専門家の医療診断・治療技能とは何か?』という本研究の核心をなす問いに極限まで深く迫ってゆくことや医療診断・治療の生産性向上が可能になるものと期待されている。
上記の成果に関連してAI・ロボティック支援医療診断・治療システム分野の一流国際誌(IJCARS)への論文掲載、トップカンファレンスでの発表(CARS)、MEDTEC Japan 2022、23への出展、日経Tech Foresight、日経クロステック等への掲載など,医療診断・治療のための生体患部抽出・追従・モニタリング技術のパイオニアかつ中核的な存在として国内外からきわめて高い注目を集めてきている。

1: Research has progressed more than it was originally planned.

Reason

本研究プロジェクトは当初の計画を超過達成しながら進展している。とりわけ、以下の点については顕著な進展があった。
(1点目) 単に臓器内に埋め込まれた患部を抽出・追従・モニタリングするのみならず、追従中に獲得した医用画像の適正度や同一断面性を評価するアルゴリズムを開発するなど、顕著な成果があった。
(2点目)対象臓器と音響陰影の重複を深層学習およびロボット技術の援用により、自動的に回避するロボット制御アルゴリズム、ロボットによる診断画像の自動獲得アルゴリズムの開発において顕著な成果があった。
(3点目)(コア技術I)に関して、本プロジェクトにより世界で初めて超音波診断・治療ロボットのみならず,診断対象である患部の埋め込まれた患者ロボットのプロトタイプをも統合的に構築したシステムが具現化された。この世界で初めて具現化された『超音波診断・治療ロボットおよび診断対象である患部の埋め込まれた患者ロボットの統合ロボティック・システム』という、極力あえて人間を介さない統合システム上において今後、デジタルに実装される機能関数群の構造によって構成論的に『医療専門家の医療診断・治療技能とは何か?』という本研究の核心をなす問いに極限まで深く迫ってゆくことや医療診断・治療の生産性向上が可能になるものと期待されている。

Strategy for Future Research Activity

本研究課題の目的は胸腹部の医療診断・治療を対象として、人工知能技術・ロボット技術・超音波技術を基盤にロボティック超音波診断・治療基盤システムの構築法を確立することであり、安全・安心に動作するシステムを実現するとともに、システムの有効性および安全性を実証する。
この研究目的を達成するために以下の5つの作業手順を順次遂行することで医療技能を機能として抽出、構造化し、デジタル機能関数としてシステムの機構・制御・画像処理アルゴリズム上に実装(医療技能のデジタル化)する:(1)医療診断・治療技能を機能として抽出・構造化、(2)機能におけるパラメータ解析、(3)機能の設計指針化、(4)機能の実装、(5)実験による機能の評価・改良。
本研究プロジェクトでは今後、(5)のステップを中心に研究を遂行する:(5)機能の評価・改良:機能の信頼性を確保するため、基礎実験・検証を通して機能の評価を行なう。評価結果は設計指針にフィードバックされ、これをもとにシステムの改良を行ない、システムの機構・制御・画像処理アルゴリズム上で機能の高度化を図る。その際、必要ならば専門医の医療技能に啓発された全く新しいアプローチから機能を追加・実装することにより、さらなる医療の質の向上(高速・高精度化)を図る。
なかでも本プロジェクトにより世界で初めて超音波診断・治療ロボットのみならず,診断対象である患部の埋め込まれた患者ロボットのプロトタイプをも統合的に構築したシステムが具現化された。この世界で初めて具現化された、極力あえて人間を介さない統合ロボティック・システム上において今後、デジタルに実装される機能関数群の構造によって構成論的に『医療専門家の医療診断・治療技能とは何か?』という本研究の核心をなす問いに極限まで深く迫ってゆくことや医療診断・治療の生産性向上が可能になるものと期待されている。
