敗血症は致死的病態であるだけでなく、生存回復しても、日常生活に支障をきたすような機能障害を残す患者が多いことが問題である。その原因は敗血症により引き起こされた長期にわたる概日リズムの乱れが一因であると考えられている。われわれは概日リズムの是正により認知機能障害を治療できるのではないかと仮説を立てた。“光”は概日リズムを形成する重要な調整因子である。本研究ではマウス敗血症モデルに対し概日リズム是正を目的とした光治療を行い、概日リズムと敗血症後の認知機能障害との関係を明らかにするとともに、光治療の臨床応用につなげることを目標とする。

| Outline of Final Research Achievements |
This study investigated whether correction of circadian rhythm disruption via high-intensity light exposure exerts neuroprotective effects against cognitive impairment following sepsis. In a murine sepsis model induced by intraperitoneal injection of cecal slurry, expression of the proinflammatory chemokine CXCL1 was markedly elevated in the hippocampus. Notably, high-intensity light exposure significantly suppressed CXCL1 expression, indicating attenuation of neuroinflammation. Behavioral analyses demonstrated preserved cognitive performance in the high-light group. Furthermore, the expression rhythms of circadian clock genes (Per2, Bmal1, and Nr1d2) were restored toward control patterns. These findings suggest that circadian rhythm correction through photic stimulation may have therapeutic potential for both neuroinflammation and cognitive dysfunction associated with sepsis.

| Academic Significance and Societal Importance of the Research Achievements |
本研究は、敗血症後に高頻度で生じる認知機能障害に対し、非薬物的かつ非侵襲的な介入である高照度光照射が有効である可能性を示した点で学術的意義を有する。特に、光刺激によって概日リズム関連遺伝子の発現リズムが改善され、神経炎症の抑制や認知機能の維持が確認されたことは、光による生体調節機構の重要性を再認識させる知見である。また、光療法は安全性が高く臨床導入のハードルが低いため、新たな介入法として社会的意義も大きく、集中治療後の長期的なQOL向上や医療・介護負担の軽減につながることが期待される。
