Outline of Research at the Start

排尿障害は神経変性疾患など中枢から末梢に至る神経因性の障害から下部尿路障害に至る様々なレベルで障害される為、その障害像を明らかにすることが重要である。そこで、排尿を指令する脊髄の中枢神経機構に着目し、未だ不明な正常排尿時の神経活動の実体や排尿脊髄回路の役割を、世界に先駆けて独自に開発した生理学的手法を用いた統合的研究により明らかにする。

Outline of Annual Research Achievements

In vivo標本を用いた排尿の脊髄神経機構の解析を前年度から継続して遂行し、特に脊髄損傷モデルを用いた実験・解析を進め、正常から得られた成果と比較した。また、人為的活動操作を行う動物を作出し、排尿や神経活動への影響を検討した。行動・排尿量の解析から、脊髄損傷モデルでは排尿回数が増加し、排尿量は減少した。脊髄副交感神経核の神経活動と膀胱内圧を同時に記録すると、自発性発火が正常と比して多く観察された。また、膀胱内圧の一過性の上昇が頻繁に起こり、脊髄副交感神経核のニューロン活動も一過性に増加するなど、脊髄神経の発火や興奮性は維持されていた。しかしながら、排尿回数は減少し、排尿量も減少した。次いで、ウイルスベクターを用いて人為的活動操作するための動物を作出した。Cre依存性にGABAニューロンにハロロドプシンを発現させた動物から、in vivo脊髄細胞外記録を行ったが、光刺激に応じた神経活動の著明な抑制が観察されず、有効な活動操作は困難であり、白質など表面から光を有効に透過させることが困難であることが原因と考えられた。そこで、Cre依存性にGiタンパク質を発現させるウイルスベクターを作成し、DREDDs法により人為的活動操作する動物を作出した。この動物では人工リガンドの投与により排尿や神経活動への影響が観察され有用なモデルであることが示された。しかしながら、同時に自発性の疼痛が発現するなど、排尿機能以外にも影響が現れた。以上より、脊髄損傷時には、節前ニューロンの活動は意外にも維持されており、膀胱への指令は十分であること、また、抑制系の人為的活動操作は膀胱機能の回復に有用であることが明らかとなった。本研究から得られた脊髄神経機構は、今後の排尿を回復する臨床応用研究への展開や、排尿を回復させる治療法の開発に有用であり、今後の更なる研究展開が期待された。
