Outline of Research at the Start

高速撮像下(数1,000 fps 超)における体表近傍(深さ約4 cm 未満)の脈管および周辺軟組織の機能および質的評価を可能とする信号解析技術を開発し,臨床データにおける有効性を評価する.高速撮像化を考慮した超音波計測および解析法の改良を検討し,ファントム実験において従来法と比較する.さらに,マクロな生体圧変動や流体力学的特性と微小散乱体の物理特性の関係性の解明を目指し,圧力変動・流体を考慮したエコーシミュレーションでの検証を行う.

Outline of Annual Research Achievements

昨年度までに構築した計測系を用い,臨床データにおける解析条件を想定した処理手法の効果検証を行った.具体的には,周囲組織や血管壁からの定在波成分(クラッタ信号)が混在するため,血液からのエコー信号を強調するフィルタリング処理が必須である.本項目では,クラッタ信号の強さが異なるファントムを対象として,血球凝集解析におけるフィルタリングの効果を実験的に検証した.結果,クラッタ信号の影響で減衰した微弱な血液エコーにおいて,フィルタを考慮した解析により,クラッタ信号の影響が低い状態と同等の精度で評価できる可能性を示唆した.
昨年度までに,直径5~40 umのポリアミド粒子を単体~凝集・凝固した赤血球として模擬し,粒径分布が異なる流体中の超音波伝搬方向の周波数特性を解析してきた.これらの流体では壁ずり応力に依らず粒子の特性は一定であるため,ニュートン流体として振舞う.しかし,実際の血液では血しょう対赤血球のずり特性(血球凝集)が反映され,非ニュートン流体として振舞う.基礎実験系において血液のずり特性を反映した流体計測を行うために,食肉用ブタの血液サンプルを用いた新たな計測システムを構築した.具体的には,様々な状態にある血液を再現するための処理プロトコルの確立および安定した定常流状態での計測系を構築した.
最終的なアウトプットとして,拍動周期中の静脈の赤血球凝集程度や微小血栓数を非侵襲に計測することを目指した.金沢医科大学の八木教授のご協力の元,健常者および糖尿病などの疾患を伴う計約50例の頸静脈データを新たに収集し,項目1において最適化した解析処理を適用した.解析の結果から,特定の疾患例において1拍動周期における超音波伝搬方向の周波数特性の違いが壁ずり応力に依存した.項目2の検討においても,血球のずり特性を反映させたサンプルにおいて上記の傾向が観察された.
