Outline of Research at the Start

近年、認知症に関連するエピゲノム制御（DNAの塩基配列自体の変化を伴わず、遺伝子の発現過程を調節する仕組み）の解明に向けた研究が進められており、エピゲノムはアルツハイマー病や軽度認知障害（Mild Cognitive Impairment: MCI）発症のバイオマーカーとなる可能性が示唆されている。また、MCIは認知症への移行率が高い一方で、正常な認知機能へと改善する可能性を持つことから、認知症予防の標的集団として注目されている。本研究は、MCI改善の機序について、新規バイオマーカーとして注目されているエピゲノムの側面から解明することを目的としている。

Outline of Annual Research Achievements

近年、エピゲノムはアルツハイマー病や軽度認知障害（Mild cognitive impairment: MCI）発症のバイオマーカーとなる可能性が示唆されている。また、MCIは認知症への移行率が高い一方で、正常な認知機能へと改善する可能性を持つことが知られている。本研究は、MCI改善の機序について、エピゲノムの側面から解明することを目的とした。
最終年度では、2年目までに実施した追跡調査のデータを用いて、MCIを有する高齢者の認知機能やエピゲノムに関する縦断解析を実施した。認知機能評価では、ベースラインとフォローアップにおいてNational Center for Geriatrics and Gerontology-functional assessment tool（NCGG-FAT）を用い、単語記憶、注意・遂行機能、処理速度をテストした。エピゲノム情報については、ベースラインとフォローアップにおいて血液検体から網羅的なmicroRNAプロファイリングを行った。
ベースラインでMCIと判定された者は23名であり、そのうちエピゲノム情報のフォローアップを完遂した者は8名であった。MCI高齢者8名における認知機能の変化は、認知機能が改善しなかった群（Stable group）が5名、MCIから正常に改善した群（Reversion group）が3名であった。この2群間でmicroRNAの血中発現量の変化を比較した結果、先行研究で神経変性疾患との関連が報告されているmicroRNAの変化量に統計学的な有意差が認められた。
