Outline of Research at the Start

日本に約1万人存在する大腿切断者のうち,約半数が年間で1回以上の転倒を経験するという事実は,20年間ほとんど変わっていない.彼らが歩行中に転倒を引き起こす要因のひとつとして,全身動作の不安定性があげられる.通常,ヒトの歩行においては,身体を構成する各体節が互いの角運動量を相殺することで,全身の安定性を保っている.しかし,左右非対称な脚および歩容を有する片側大腿切断者にとって,歩行中に全身の角運動量を調整し,安定性を維持することは非常に困難である.そこで本研究は,全身動作の安定性を定量評価することで,転倒ハイリスク者の早期発見および転倒予防策を講じ,20年来変わらない転倒率の低減を目指す.

Outline of Annual Research Achievements

2022年度は、臨床現場においてデータに基づいた即時介入と効果検証を行う予定であった。しかしながら、新型コロナウイルス感染症の影響により、臨床現場での計測は困難となった。したがって、2021年度までに得られた大腿義足歩行中の角運動量制御機序に関する標準データセットをベースとし、より活動度の低い大腿義足ユーザにおける歩行の動的安定性と介入効果をみることとした。活動度の低い大腿義足ユーザの歩行データセットとして、ユタ大学が公開する大腿義足歩行のパブリックデータを使用した。まずは、活動度K2レベルの大腿義足ユーザ6名のうち、トレッドミル歩行を手すりなしで行った3名と手すりありで行った3名における、荷重パターンの違いを見るため、地面反力変数の比較を行った。その結果、手すりありの大腿義足ユーザは、義足片足支持期における鉛直方向の荷重が有意に小さく、健側と義足における地面反力の対称度が大きいことが分かった。地面反力の発揮パターンが健側義足間で差が少ないほど、ステップ間における地面反力由来の身体重心まわり外部モーメントに差が生じなくなる。角運動量の変化量は、外部モーメントの時間積分値と等しいため、手すりがある場合の方が片側大腿切断者の身体重心まわりの姿勢変動が軽減されることを意味する。したがって本研究の結果から、手すりによる歩行サポートが、大腿切断者の身体重心まわりの角運動量制御に、十分な介入効果をもつことが示唆された。
