敗血症は細菌感染を主因としておこるが、いまだ有効な治療法が確立されておらず先進国でも致死率が高い疾患である。一方、最近になって細胞間コミュニケーションを媒介する細胞外小胞エクソームが、敗血症の病態において保護的に働くことが報告された。申請者はこれまでに生体防御ペプチドLL-37を敗血症モデルに投与すると、菌血症とサイトカイン生成が抑制され致死率が改善することを明らかにしている。さらに申請者は、LL-37が敗血症モデルにおいてエクソソーム放出を増加することを見出した。そこで本研究では「LL-37によるエクソソームの放出誘導」に着目して、敗血症の病態解明と、それに基づく新たな治療法の開発を目指す。

| Outline of Final Research Achievements |
In this study, we focused on "LL-37-induced extracellular vesicle (EV) release" in a sepsis model, and analyzed the function of EVs. Since LL-37 releases two kinds of EVs "exosomes" (100 nm) and "ectosomes" (200 nm or larger) from neutrophils, we compared the antibacterial effects of exosomes and ectosomes. As a result, we found that ectosomes contained antibacterial proteins and peptides derived from neutrophils and had antibacterial activity, while exosomes contained almost no antibacterial proteins and peptides and had weak antibacterial activity. This suggests that exosomes may function in sepsis through functions other than antibacterial action.
