Outline of Research at the Start

B型肝炎ウイルス(HBV)既往感染歴を有する、造血器腫瘍患者において、がん化学療法に関連するHBV再活性化後の先制ウイルス治療開始後の核酸アナログの適切な中止規準に関するエビデンスは限られている。
HBV複製基点となる、肝臓内のcovalently closed circular DNA (cccDNA)の血清サロゲートマーカーである、HBs抗原およびHBコア関連抗原の高感度系キットを用いて、HBV再活性化後の核酸アナログ中止規準の確立に資する基礎的検討を行う。エビデンスに基づく、HBV再活性化後の核酸アナログの適切な投与期間が設定できれば、医療経済への負担軽減につながることが期待できる。

Outline of Final Research Achievements

Reactivation of hepatitis B virus (HBV) after cancer chemotherapy can now be managed through preemptive risk assessment and risk-adapted strategies. However, evidence regarding the appropriate duration of nucleos(t)ide analogue (NA) therapy following HBV reactivation remains limited. In particular, HBV reactivation from resolved infection accounts for a substantial number of cases, and prolonged NA therapy imposes a significant economic burden on healthcare systems.

To address this issue, we conducted a study aimed at establishing discontinuation criteria for NA therapy after HBV reactivation using highly sensitive HBcrAg as a biomarker. Through a multicenter retrospective study, we evaluated the clinical significance of highly sensitive HBcrAg and other biomarkers (Suzuki T, J Gastroenterol. 2022; 57:486; Hagiwara S, et al. Hepatol Res. 2022; 52:745; Suzuki T, et al. Hepatol Res. 2023; 53:289; Suzuki T, et al. Ann Hepatol. 2025; 30:101764).

Academic Significance and Societal Importance of the Research Achievements

HBV再活性化後の核酸アナログ中止規準の確立のためには、ウイルス因子および宿主因子(患者の免疫状態および抗がん剤治療内容)の要因を考慮する必要がある。HBV関連高感度マーカーの開発によりウイルス要因は考慮できたが、患者の免疫状態に関するバイオマーカーの開発は急務の課題である。本研究により、HBV再活性化後のウイルスおよび宿主の免疫状態が把握できるようになれば、核酸アナログ中止規準の確立に加え、その他の分野におけるヒト免疫応答の把握につなることが期待できる。また、核酸アナログ投与期間の最適化は、患者さんのみならず国全体の医療経済への負担軽減に寄与することが期待できる。
