Outline of Research at the Start

癌組織ならびに直腸粘膜のDNAメチル化による潰瘍性大腸炎関連大腸癌の鑑別診断
潰瘍性大腸炎関連大腸癌症例と、孤発性大腸癌切除例の癌組織ならびに直腸粘膜組織を用い、NGSによる網羅的DNAメチル化解析を行うことで、潰瘍性大腸炎に合併した孤発性大腸癌症例を用いてその鑑別能を確認する。
糞便中, 循環cell free RNA・exosomal RNAメチル化解析を用いた癌合併潰瘍性大腸炎患者診断
上記の網羅的DNAメチル化解析を行い、癌合併ならびに非合併潰瘍性大腸炎患者、孤発性大腸癌ならびに健常者の便、術前血清から、候補RNAメチル化マーカーの定量を行い、癌合併潰瘍性大腸炎患者診断に有効かを検証する。

Outline of Final Research Achievements

We collected ulcerative colitis mucosa, normal colon mucosa, dysplasia, and cancerous tissue from patients with ulcerative colitis or colorectal cancer underwent surgery at our department. We performed a comprehensive analysis of candidate methylated genes involved in carcinogenesis using a large number of samples.
Gene X methylation levels were significantly higher in cancer tissues of S-CRC and UC-CRC patients. In addition, gene X methylation levels significantly higher in rectal mucosal tissue than cecal tissue. The methylation level of gene X in rectal mucosal tissue may be a predictive marker for carcinogenesis.

Academic Significance and Societal Importance of the Research Achievements

今回、同定した遺伝子XのDNAメチル化マーカーを用いたUC-CRCとS-CRCを鑑別可能とする診断マーカーを開発することで、これまでにない新しい診断治療アプローチとなる。この診断マーカーが直腸粘膜組織の発現を検討することで診断困難な潰瘍性大腸炎癌化患者を非侵襲的検査法で同定できることが期待され、さらに便中・血中の発現との関連を検討し、より少ないサーベイランス方法を確立することで、UCに合併したS-CRCに対する無用な大腸全摘術を回避し、患者の腫瘍学的予後のみならず術後QOLを担保することが可能となる。
