Outline of Research at the Start

耐糖能障害を伴う(重症)敗血症では、過剰な免疫抑制状態が生じ、予後不良を来すが、我々は血球や血管内皮細胞が産生するmicroRNA(miRNA)が、炎症病態を修飾する重要な要因になると考える。その機序と新しい治療法を見出すために、次世代シーケンサーによる網羅的なmiRNA発現解析、遺伝子導入手技等を用いて、病態を解明し、今後の遺伝子治療を展開したい。その中で、特に好中球が放出する好中球細胞外トラップ(Neutrophil extracellular traps, NETs)に注目している。

Outline of Annual Research Achievements

敗血症では免疫抑制状態が生じ予後不良を来すが、我々は血球や血管内皮細胞が産生するmicroRNAが、炎症病態を修飾する重要な要因になると考える。その機序と新しい治療法を見出すために、網羅的なmiRNA発現解析、遺伝子導入手技等を用いて病態を解明し、今後の遺伝子治療を展開したい。特に好中球が放出するNETsに注目している。
ヒト骨髄性白血病細胞HL-60細胞を、ジメチルスルホキシド(DMSO)にて好中球に分化し実験を行った。HL-60 培養細胞株を用いて、in vitro での分葉核の形成と NETs 類似の構造体の誘導系を確立した。糖濃度の異なる培養液を用いた好中球の培養系において、LPSを含有する培養液で24-48時間培養後、ホルボール12-ミリスチン酸13-酢酸塩(PMA)負荷し、好中球の脱顆粒化、NETs、ヒストンを蛍光色素染色後、蛍光倒立顕微鏡で観察、また培養後の回収液をフローサイトメーターで定量化し、対象群と比較検討した。さらに細胞からTotal RNAを分離し、次世代シーケンサーを用いてmRNA及びmiRNAの発現の網羅的解析を、CLCバイオ社解析ソフト(Genomic Work Bench)を使用して行った。特にNETsを含む、細胞死・炎症惹起物質に関連するmRNA、及びそれに相補配列のあるmiRNAを選別した。Real-Time PCR法で、RNA発現の絶対定量を行った。また、上記の実験で上昇したmiRNAのmimic(Pre-miRTM miRNA Precursor Molecules, Ambion社)と、そのmiRNAに特異的なmiRNA阻害薬(Anti-miRTM miRNA Inhibitor, Ambion社)をHL-60 培養細胞株へ遺伝子導入し、好中球の脱顆粒化、NETs、ヒストンを蛍光色素染色、フローサイトメトリーで、比較検討した。

コロナ禍で、外部委託業者の出勤時間、営業時間が限られていたため、一部の実験に遅れが生じている。

Strategy for Future Research Activity

(臨床研究)ICU入室患者を対象に、1)非敗血症患者、2) 耐糖能障害の無い敗血症患者、3) 耐糖能障害の有る敗血症患者から採血後、好中球を抽出し、細胞培養研究で検討した好中球の脱顆粒化、NETs、ヒストンの発現、及びmiRNA発現のデータの比較検討を行う。
