Outline of Research at the Start

本研究では、脳虚血や認知機能低下におけるくも膜の炎症線維化という様相とそれに基づ く論理を検討する。具体的には、実臨床で得られた検体において、くも膜内の線維芽細胞を 制御する炎症マーカーや線維芽細胞に分化する間葉系幹細胞を見出し、画像上の脳小血管病 や認知機能との関係を明らかにする。さらに、ラット慢性脳虚血モデルを作成し、くも膜の 線維芽細胞の分化増殖機序とそれらの制御機序を見出し、くも膜の炎症線維化制御というダ イナミックな病態と治療効果の可能性を明らかにする。

Outline of Final Research Achievements

The arachnoid membrane on the frontal cortex obtained from patients with chronic cerebral ischemic disease and non-ischemic disease were analyzed. The thickness of the arachnoid membrane was influenced by age. In thicker membranes, fibroblasts and macrophages were increased in the inner layer, along with an increase in inflammatory markers such as VEGFα and TGFβ. Additionally, the presence of neovascularization within the arachnoid membrane was confirmed. In ischemic diseases, a strong inflammatory response was observed, indicating the impact of the disease. Furthermore, in experimental animals, bilateral occlusion of the common carotid arteries was performed, and seizure waves and power spectrum analysis were conducted. It was discovered that ischemia has an impact on the occurrence of seizures. In seizures associated with reperfusion injury, it was found that microbleeds have a significant influence.

Academic Significance and Societal Importance of the Research Achievements

近年、脳小血管病の発現に脳内の老廃物を排出するglymphatic systemが関係しているという報告も散見されており、認知症との関連が示唆されている。本研究は、脳虚血に対してくも膜の炎症や線維化制御に着目したはじめての研究であり、微弱な頭蓋内慢性炎症がもたらすくも膜の炎症や線維化が脳小血管病や頭蓋内血管病変を通して脳機能に及ぼす影響を解明することに貢献した。このメカニズムを通した治療が実現することにより、拡大増加する医療費や介護負担の縮小をめざすことができ、今後の超高齢社会への対応策となると考えている。
