| Outline of Research at the Start |
依然致死的な中枢神経系原発悪性リンパ腫（PCNSL）の病態解明と治療開発のため、分子遺伝学的予後因子の特定が重要である。従来の後方視的PCNSL予後因子研究結果は一定せず、背景因子や治療内容の多様性が原因と考えられる。JCOG1114C第Ⅲ相試験はPCNSLに対して大量methotrexate（MTX）療法+全脳照射にtemozolomide（TMZ）の上乗せ効果を比較検証する試験である。本研究ではこの臨床試験の均一な集団症例から、標的シークエンス、MSP法ならびにNanoString法による遺伝子解析により、PCNSLの予後因子ならびにMTX、TMZ反応性規定因子を検証する事を目標とする。

| Outline of Final Research Achievements |
This study aims at identifying molecular prognostic factors among patients enrolled in the JCOG1114C phase III clinical trial. In this year, we performed MGMT promoter methylation analysis in order to investigate its potential role as a determinant for treatment effect of temozolomide (TMZ). As a result, MGMT promotor methylation was not a prognostic factor among all analyzed patients in this study. Moreover, no OS benefit of temozolomide was observed in either MGMT hyper/hypomethylated groups. These results indicate that MGMT promoter methylation was not predictive nor prognostic among patients treated in the JCOG1114C trial.

| Academic Significance and Societal Importance of the Research Achievements |
分子遺伝学的予後因子が依然として十分に明らかになっていない中枢神経系原発悪性リンパ腫（PCNSL）に対し、治療方法や患者背景が均一である第Ⅲ相臨床試験の患者集団で、MGMT遺伝子 プロモーター領域のメチル化が予後因子でなく、またTMZの治療効果予測因子でもなかったことが示された。原発性悪性脳腫瘍の主要な疾患である膠芽腫では、MGMTメチル化状態はTMZの予測因子であることから、PCNSLが同じ悪性脳腫瘍としても膠芽腫とは異なる病態や治療反応性をもつ疾患であることが明らかとなり、またTMZのPCNSLでの有用性を問うJCOG1114C試験の結果の解釈に重要な科学的裏付けを付加することに寄与した。
