本研究では凍結処理を行った腫瘍骨移植の前後において、癌に対する免疫賦活化のモニタリングを行い、凍結免疫の効果につき検討を行うことである。さらに現在適応となっている免疫チェックポイント阻害薬に対する上乗せ効果を調査する。

| Outline of Final Research Achievements |
Cryotherapy can stimulate immune responses and induce systemic antitumor effects, the abscopal effect. We developed a new technique for spinal bone tumors that involves using frozen tumor-containing autologous bone for anterior spinal reconstruction following total en-block spondyrectomy (second-generation TES), aiming to activate cryoimmunity. We focused on analyzing changes in the T cell receptor (TCR) repertoire after surgery to evaluate the diversity of T cells. Blood samples were collected pre- and post-operatively, and RNA extraction and immune-sequencing were performed. Compared to pre-surgery samples, the diversity and abundance of the CDR-3 regions of the TCR α and β chains decreased, suggesting that more selective clones may have emerged and influenced immune responses. This study demonstrated that the transplantation of frozen tu-mor-containing autologous bone has an impact on the immune system through TCR repertoire analysis.

| Academic Significance and Societal Importance of the Research Achievements |
本研究では、凍結療法が免疫反応を刺激し、全身的な抗腫瘍効果を引き出す可能性が示唆されました。転移性骨腫瘍は、治療が困難な病態ですが、免疫を積極的に活性化させる新たな治療戦略の開発が期待されます。本研究で用いた、レパトア解析は免疫学の理解を深め、患者ごとの免疫応答の違いを把握する手段としても重要であり、免疫動態を理解し、将来の免疫療法や新規治療の基礎となる知見を提供しており、医療の革新や患者のQOL向上に寄与することが期待されます。
