頭頸部がん放射線治療時には口腔粘膜炎が出現する。照射単独の場合はデキサルチン軟膏塗布によりグレード3の重症口腔粘膜炎の発症を予防することは可能であるが、シスプラチンやセツキシマブとの併用による化学放射線治療ではデキサルチン軟膏の効果はみられないことを以前に当教室より報告した。そこで、より強度の高いステロイド軟膏であるリンデロンVG軟膏を化学放射線治療時に使用することを計画した。頭頸部がん化学放射線治療患者においてグレード2の口腔粘膜炎が出現した時点でデキサルチン軟膏投与群とリンデロンVG軟膏投与群に割り付け、グレード3の口腔粘膜炎発症をアウトカムとするランダム化比較試験を行う。

Oral mucositis almost inevitably develops when radiotherapy is performed for head and neck cancer in which the oral cavity enters the irradiation field, but there are few effective methods for its prevention and treatment. There are some reports on the preventive effect of medium-class steroid ointment, but it is not highly effective against oral mucositis during radiotherapy combined with anticancer drugs and molecular-targeted drugs. Therefore, we decided to investigate the effectiveness of strong class steroid ointment.
The study was designed as a multicenter, specific clinical study. It was decided to conduct the study with the cooperation of seven facilities, but the start of case registration was delayed due to the impact of COVID-19, and only about 60% of the target number of cases has been registered, so the research period has been extended. continuing.

| Academic Significance and Societal Importance of the Research Achievements |
頭頸部がんに対する放射線治療時には重篤な口腔粘膜炎がしばしば出現し、患者のQOLの低下や放射線治療の中断につながることがある。しかしその予防法については確立していない。
今回の研究はstrong classのステロイド軟膏により放射線性口腔粘膜炎の重症化を予防できるかどうか、またステロイド軟膏により口腔の細菌叢が乱れ口腔カンジダ症発症を促進しないかどうかを調べる多施設共同ランダム化比較試験である。本研究により放射線性口腔粘膜炎重症化の予防法が確立すれば、患者の苦痛軽減や治療効果の向上につながることが期待でき、学術的意義、社会的意義は大きい。
