| Outline of Research at the Start |
肥満に関連した保健医療介入の費用対効果を比較するために必要な、日本人肥満者の総合的な体格(BMI)ごとの効用値(医療経済評価に利用可能な選好に基づくQOL尺度)のセットが、現在わが国に存在しない。 そこで本研究では、①日本肥満症治療学会の協力の下で収集が行われている高度肥満患者の効用値と、②本研究で収集する、一般的に肥満と呼ばれる者(BMI≧25)へのインターネット調査で得る効用値、の2つのデータを用いて、合併症を含むさまざまな状態を含む、BMIによる効用値のインデックス(データの一覧表)の作成を試みる。

| Outline of Final Research Achievements |
In response to the global increase in obesity, including in Japan, we analyzed the relationship between BMI and health-related quality of life (QOL) to evaluate the cost-effectiveness of obesity treatments. In 2023, we conducted a web-based survey targeting adults aged 25 and over who had a history of medical visits for obesity or who were concerned about obesity. Using analyses with EQ-5D and SF-36, we found that QOL was higher in the normal-weight and grade 1 obesity groups, however decreased in grade 2 and higher obesity groups. In SF-36, the physical component score declined linearly as BMI increased. Mediation analysis showed that the decline in health-related QOL associated with obesity was mainly due to its impact on physical health. Although comorbidities also affected QOL, in grade 2 and 3 obesity, obesity itself had a direct negative impact. These findings were consistent with results from studies in other countries.

| Academic Significance and Societal Importance of the Research Achievements |
本研究は、日本人の肥満治療の費用対効果評価に不可欠なBMI別効用値インデックス作成を試みたが、サンプル等から肥満度による分析を行った。しかしながら、本研究で肥満2度以上でQOL低下が合併症の影響を除く直接的な生理的影響によることを実証し、国際的な知見と共通であることを示した。社会的には、本研究で得られたBMI別の効用値は、肥満関連疾患の予防や治療介入の費用対効果分析における参照値分析の予備的な知見となる。これは、限られた医療資源の効率的な配分に寄与することが期待される。
