| Outline of Research at the Start |
研究の基本デザインはコホート研究である。季節ごとの1週間当たりの入浴頻度と要介護状態の新規発生、認知症の新規発生、抑うつ状態の新規発生の縦断的な関連について、性別、年齢、その他の交絡因子を調整して分析し、その影響を明らかにする。 日本老年学的評価研究(JAGES)の一貫として、2010 年度に北海道から沖縄まで全国28 市町村の要介護認定を受けていない在宅の65 歳以上の高齢者16 万9 千人を対象に自記式郵送調査を行い、11万2 千人の回答(回収率66.3%)を得た。このデータを基本的にベースラインデータとして要介護状態の新規発生、認知症の新規発生、抑うつ状態の新規発生追跡調査を実施する。

| Outline of Final Research Achievements |
As part of the Japan Gerontological Evaluation Study (JAGES), we conducted a multivariate analysis of the association between the onset of depression and seasonal tub bathing over a six-year follow-up period in approximately 3,200 people and determined odds ratios. The odds ratios of developing depression for bathing in the bathtub seven or more times per week compared with bathing in the bathtub 0-6 times per week were 0.84 in summer and 0.76 in winter, indicating that bathing in the bathtub seven or more times per week in winter was associated with a significantly lower risk of developing depression. Similarly, when 7,500 people were followed for 9 years for the development of dementia, the sub-hazard ratios were 0.74 in summer and 0.82 in winter, indicating that bathing in the bathtub seven or more times a week in summer was associated with a significantly lower risk of developing dementia.

| Academic Significance and Societal Importance of the Research Achievements |
高齢化が急速に進展する中、高齢者のメンタルヘルスや認知症予防は喫緊の課題である。浴槽は多くの一般家庭に備えてあり、浴槽入浴は我が国では多くの者が行っている生活習慣であるが、これまで浴槽入浴の長期的健康効果はあまり検討されてこなかった。本研究では、長期的追跡研究の結果、関係する交絡因子を調整後であっても毎日浴槽に浸かって入浴することで抑うつの発症リスクが24%減少することが分かった。また同様に認知症発症のリスクを26%減少することが分かった。浴槽入浴を実施することで高齢者の抑うつや認知症発症を予防する可能性があることが示唆された。手軽な健康生活習慣として浴槽入浴を活用しても良いかもしれない。
