| Outline of Research at the Start |
筋委縮性側索硬化症(ALS)療養者は、進行性の運動ニューロン障害により生活行動が徐々 に縮小し、最終的には呼吸筋麻痺のため呼吸療法を行わなければ呼吸停止をきたす。呼吸療法の選択は生命に直結しており、ALS療養者が望む人生を選択できるよう支援する看護師の役割責任は非常に大きいが、その支援への困難感が指摘されている。そこで本研究の学術的問いを、看護師の①ALS療養者の呼吸療法の意思決定支援行動とは何か、②意思決定支援行動はどのように指標にすることができるかであり、ALS療養者の呼吸療法の意思決定支援行動の指標を開発することを目的とし、調査を実施する。

The decision-making support provided by visiting nurses for respiratory therapy for ALS patients had a seven-factor structure, starting with "assessing respiratory symptoms and formulating a policy," followed by service coordination and the establishment of a care system, shaking up the patient's resolve, and through this interaction the visiting nurses reflected on their care, returning to "assessing respiratory symptoms and formulating a policy."Within the limited consultation time, outpatient nurses "provided information tailored to the progression of the disease, taking into account the shock of the disease name," "encouraged questions based on the patient's and family's expressions and state," "assisted the attending physician's explanations using easy-to-understand words and items used," "adjusted consultation dates based on respiratory symptoms," and "connected the patient to a social worker so that social resources could be utilized."

| Academic Significance and Societal Importance of the Research Achievements |
意思決定は、一度決めれ確固となるものではなく、その時の状況などによる揺れ動くものである。ALS療養者の呼吸療法の意思決定支援が構造化できたことで、看護師として的確な評価を基に、支援をつないでいることが具体的に示され、新人期にある看護師にとって、ALS療養者に初めて関わるさいの支援の糸口とすることできる。また、療養者・家族が医療や社会サービスを意思決定していく機会は多く、神経難病に限らず、小児から高齢者、がんや認知症など、多様な疾患を抱える療養者の意思決定支援の一助になりえると考える。
