| Outline of Research at the Start |
我が国の高齢者人口は増加し，独り暮らし世帯も増えている。また，高齢者を取り巻く医療環境は，平均在院日数の減少，在宅医療への推進と変化している。そのため，ストーマの受容やストーマケアが未確立のまま退院後の生活が始まる独居高齢者が増加することが推察される。
本研究は，ストーマを造設した独居高齢者が病院から自宅にもどり新たな生活を組み立てる過程を明らかにし，支援体制を整えることを目的としている。これにより，病院と患者の生活の場である地域との連携を考えるうえで新たな知見を示すことができ，今後の地域包括ケアシステムの充足に向けた示唆をもたらすことができると考える。

| Outline of Final Research Achievements |
This study aimed to clarify the nursing practices of certified skin and wound care nurses and home care nurses in supporting the transition to home care for elderly individuals living alone who have undergone stoma creation.The nursing practices of certified nurses in skin and wound care involved predicting future needs from the hospital stay and providing continuous support tailored to each individual elderly person living alone based on comprehensive information. The nursing practices of visiting nurses involved collaborating with other professionals to provide support from multiple perspectives, taking into account the diverse living environments of elderly people living alone and the wishes of their family members living far away, thereby creating an environment where they can live in a way that suits them.

| Academic Significance and Societal Importance of the Research Achievements |
高齢者人口が増加し一人暮らし世帯も増えている日本において、地域で自分らしく生活できる環境を整えることは重要である。特に、医療的処置を必要とする高齢者の場合は退院直後より自身または他者による医療的処置への対応が必要となる。本研究の成果を日々の看護実践や病院と患者の生活の場である地域との連携において活用することにより、医療的処置を必要とする高齢者が地域で自分らしく生活することを支える一助となると考える。
