本研究は、健康の社会的決定要因により複雑な問題を抱える住民について、問題の発見から支援体制構築までの多職種連携による支援の体系化を試み、その結果をもとに「健康格差により多問題を抱える住民支援のための多職種連携モデル」を構築する。このモデルは、専門職の人材育成や住民支援の評価に活用しうるモデルであり、このモデル活用により健康格差による問題を抱える住民の支援が効果的に行われ、地域住民の健康に寄与することが期待できる。

| Outline of Final Research Achievements |
This study aims to systematize support for residents with multiple problems due to health disparities, and to construct a highly effective "multi-professional collaboration model for supporting residents with multiple problems due to health disparities."
At first, we conducted a literature review on support for cases of people with multiple problems due to health disparities, and organized the current state of research and issues. Next, we collected data through interviews on the support provided by public health nurses to residents in situations of health disparities, and conducted a qualitative analysis. Based on the results, we conducted a survey of municipal public health nurses about the support provided to residents with multiple problems due to health disparities. We are currently creating evaluation indicators for support for residents.

| Academic Significance and Societal Importance of the Research Achievements |
文献検討により、困難事例の特徴や今後の課題を明らかにした。また、質的記述的分析により、市町村保健師が遭遇する「支援困難事例」の特徴を明らかにした。さらに、49項目の支援内容について明らかにし、この項目について全国の市町村保健師の協力を得て調査を行った。
現在、有効回答300について分析中であり、市町村保健師が健康格差により多問題を抱える住民に対して行う支援項目を特定する。この結果を基に、多問題を抱える住民支援のための多職種連携モデルを構築し、住民支援に対する支援の評価や人材育成への活用について検討を継続する予定である（25K14152）。
