Outline of Research at the Start	肝細胞癌は根治切除後も高率に再発を来すことが知られている。根治術後の転移再発は予後不良であり、その機序解明と制御方法の確立が急務である。転移形成には循環腫瘍細胞が寄与していると推定されるが、肝細胞癌における循環腫瘍細胞の研究、特にその発生機序および免疫逃避機構に関しては不明な点が多い。本研究は肝細胞癌患者における循環腫瘍細胞とprogrammed cell death-ligand 1の解析をすることで、癌細胞の転移機序および免疫回避機構の解明と制御を目指すものである。
Outline of Final Research Achievements	We detected and analyzed circulating tumor cells (CTC) in hepatocellular carcinoma (HCC) by targeting glypican-3, which is specifically highly expressed in HCC. This study showed that anatomical hepatectomy should be performed in cases with high CTC levels, even if the hepatocellular carcinoma is less than 3 cm in size. Furthermore, in a study of cases in which anatomical hepatectomy was performed, CTC in the portal vein were found to be a prognostic factor.
Academic Significance and Societal Importance of the Research Achievements	切除可能な肝細胞癌に対して、系統的切除か非系統的切除かの術式選択においてCTCの有用性を示すことができた。また本研究では、門脈血中のCTC測定の有用性を示すことができ、将来的に肝細胞癌の再発制御につながる可能性が考えられた。
