好酸球性鼻副鼻腔炎は、ステロイド以外に有効な薬物療法がなく、手術しても再発しやすい難治性疾患である。現在、上皮細胞由来サイトカインや2型自然リンパ球をはじめとした自然免疫が病態形成に関わっていることが分かっている。一方、生体内には免疫抑制機構の存在が想定されているが、好酸球性鼻副鼻腔炎の病態での役割は未解明である。 本研究では、好酸球性副鼻腔炎での免疫抑制機構について、特に制御性T細胞や、抗炎症作用を有する IL-27、抑制系サイトカインであるIL-10、IL-35、TGF-βの機能を明らかにする。免疫抑制機構をターゲットとした新たな治療手段の開発につなげることを目的とする。

Outline of Annual Research Achievements |
ST2(IL-33受容体)とFoxp3の発現により、ST2+ Tregsの存在が知られている。IL-33の存在下では、ST2+ Tregsは免疫抑制活性を失い、Type2炎症を誘発する。好酸球性副鼻腔炎におけるTregsの役割についてはあまり検討されていない。 Th2細胞、ST2+TregおよびTr1の役割を調べるために、好酸球性副鼻腔炎および非好酸球性副鼻腔炎患者の鼻茸におけるTh2 (CD4+ST2+) 、Foxp3+ Treg (CD4+CD25+Foxp3+) 、ST2+Treg (CD4+CD25+ST2+Foxp3+)およびTr1 (CD4+CD25-Foxp3-IL-10+)の割合を比較検討した。Th2細胞の割合は、非好酸球性副鼻腔炎患者のものと比較して好酸球性副鼻腔炎患者で有意に増加していた。CD4+ T細胞中のFoxp3+ Tregの割合は差がなかったが、ST2+Tregの割合は、非好酸球性副鼻腔炎患者の鼻茸およびコントロール被験者の鈎状突起のものと比較して、好酸球性副鼻腔炎患者の鼻茸で有意に増加した。好酸球性副鼻腔炎患者の鼻茸では、Foxp3+ Treg細胞中のST2+Tregの比率が有意に上昇し、Foxp3+ Treg細胞の60%以上がST2+Tregで占められていた。一方、好酸球性副鼻腔炎患者の鼻茸では、非好酸球性副鼻腔炎患者の鼻茸およびコントロール被験者の鈎状突起と比較して、Tr1の陽性率が有意に減少していた。 好酸球性副鼻腔炎患者のPBMCでは、rIL-33との培養後にST2+Tregの割合が増加したが、対照被験者からのPBMCではrIL-33はST2+Tregの誘導に影響を示さなかった。好酸球性副鼻腔炎における、ST2+Treg の増加とTr1の減少は、過剰な2型炎症の抑制機構が損なわれている可能性がある。
