| Outline of Research at the Start |
患者・家族が在宅療養を意思決定するにあたり、病棟看護師の退院支援の重要性が高まっている。しかし、現場で利用可能なサービスや地域ケアの現状等の知識を看護職が得る機会は少ない現状である。本研究は、これまで1県内において病院看護職を対象に行ってきた退院支援を担う看護師の育成プログラムの評価や、修了生からのニーズ調査をもとに、地域や病院の実情に応じて組み合わせ使用可能な研修プログラムパッケージを開発する。Web公開や公開コンサルテーションシステムを用いて普及させ、退院支援を担う看護師の普遍化と底辺拡大を目指す。また、急速に高齢化が進む中国の将来を見据えたモデル事業として1大学附属病院での試用を提案する。

| Outline of Final Research Achievements |
The increasing focus on comprehensive community and home medical care highlights the crucial role of ward nurses in providing effective discharge support. However, existing training programs often fail to address the knowledge gap concerning available services and the community care landscape. To bridge this gap, principal investigators evaluated current discharge support training for hospital nurses in a specific prefecture. Their goal was to develop tailored training programs that consider the diverse challenges nurses face in discharge planning and support, ultimately enhancing patient outcomes and empowering nurses to deliver more informed care.

| Academic Significance and Societal Importance of the Research Achievements |
県内の病院看護師を対象に退院支援を担う看護師の育成教育プログラムを5年間続け、一定の成果を上げている。一方で課題もある。①実習という形態を組み入れるにあたっての調整の煩雑さ②病院業務が多忙な看護職にとって少人数の派遣にならざるをえないこと③全体に浸透するには継続と時間が必要であること④修了後の経験の蓄積に関する課題などである。本研究は、地方大学が中心になって行ってきた1県内での研修プログラムを、①～④の主な課題に対応すべく、地域や病院の実情に応じて使用可能な形にパッケージ化し、病院における地域完結志向の普遍化を目指すものである。
