本研究では、①モデル地域において実施したフレイル早期発見・予防プログラムの結果からスクリーニン グされたフレイル高リスク者を、KDBデータを含めた解析により対象者の特性の明確化・類型化を行う。②類型化に従って既存事業や専門職の介入が必要な医療・保健・介護サービ スの支援につなげる。③②の支援システムにおける促進・阻害要因を明らかにする。④②の支援システムの導入効果⑤③④の結果より、実施自治体担当者に向けたマ ニュアルを作成⑥作成したマニュアルを全国のフレイル早期発見・予防プログラム実 施自治体(地方都市、小規模都市等)へ導入し、実施可能なフレ イル支援のデファクトスタンダードの構築

| Outline of Final Research Achievements |
This study developed and evaluated an integrated support system combining public health and long-term care prevention services. By providing feedback on health checkup results from a frailty perspective, residents were encouraged to understand their health status, undergo frailty screening, and, if identified as high-risk, were connected to support through a stepwise approach. This method lowered psychological barriers and increased participation. Statistical analysis showed that frail individuals had significantly higher medical and long-term care costs, suggesting that early detection and intervention for frailty may help reduce social security expenditures.

| Academic Significance and Societal Importance of the Research Achievements |
本研究は、保健事業と介護予防事業を一体的に連携させ、地域高齢者のフレイル予防を実現するための支援モデルを構築・検証した点である。本研究では、後期高齢者健診や「通いの場」を通じて得られるデータを活用し、リスクの可視化と段階的な介入を組み合わせることで、支援接続の実効性を高める実証的成果を挙げた。 特に、①後期高齢者の質問票(QMCOO)を活用した介護リスク評価の予測妥当性の検証、②フレイル該当者と医療・介護費用との関係性を統計学的に明らかにしたこと、③住民の行動変容に着目した段階的支援プロセスの有効性の実証は、保健・医療・介護領域を横断する学術的知見として高く評価される。
