悪性神経膠腫新規治療法選択に必要な病理診断に関する研究

Research Abstract

組織多様性を示す神経膠腫の各組織型におけるO^6-methylguanine-methyltransferase (O^6-MGMT)をはじめとする治療反応性因子を検索し、temozolomide (TMZ)の治療効果を増強する投与法を検討した。TMZおよび放射線治療の標準治療が計画された初発悪性神経膠腫78症例に関してO^6-MGMTのメチル化および1p/19qのloss of heterozygosity (LOH)の有無に応じて、TMZの標準治療のみあるいはInterferon-β (INF-β)やprocarbazine (PCZ)の併用療法を施行した。再発時に手術された25例に関しても同様の遺伝子解析とともに、β-galactosidaseをはじめsenescenceのマーカーを検索した。Pseudopalisadingの部分やoligodendroglial componentでO^6-MGMTのメチル化および1p/19qのLOHがみられた19例では、TMZの標準治療のみで無増悪生存期間(PFS)および全生存期間(OS)ともに有意に延長し、奏効率も良好であったが、INF-βを併用した2例とPCZ併用の1例ではPFS、OSともに不良であった。再発手術症例におけるO^6-MGMTのメチル化や1p/19qのLOHの状態は初発時と変化がなかったが、senescenceのマーカーの発現が増加している症例ではTMZの継続により、PFSやOSの有意の延長がみられた。初発および再発悪性神経膠腫に対するTMZの効果を高めるためにはsenescenceに関わる因子も含めた複数の因子の検索が必須であり、TMZ反応性・不応性症例ともにO^6-MGMTの枯渇化やsenescenceの誘導などTMZの作用を増強する必要性が示された。
