Research Abstract

研究期間開始後、治療を予定されている複数の担がん患者に対して文書で同意の上、F-18FMISO(F-18で標識されたフルオロミソニダゾール)を用いた全身PET(ポジトロン断層撮影)検査を施行した。使用機器は最新のPET-CT機器または半導体PET用いて行った。当施設における薬剤の合成法については研究分担者久下らにより確立、安定供給可能となった。
次に文献等から、撮像プロトコールを決定し検証を続けた。研究対象にはこれまで特記すべき有害事象は起こっていない。
被験者の個人情報を特定されない様処置して基本データベースを作成した。そのうえで各症例の画像評価を行い、特に癌部FMISOの集積(腫瘍内低酸素領域)を解析評価した。また他モダリティ(特に半導体PET)、特にF-18FDG(F-18で標識されたフルオロデオキシグルコース)との比較を詳細に行っているところである。代数的・視覚的・統計的各方面からのアプローチを模索した。
また将来放射線治療計画が飛躍的に進歩するのを見越し、難治癌である膵臓癌の術前化学放射線治療前後のPET評価と予後を追跡した報告を米国の学会で行った。
さらに研究を加速させるために、研究分担者久下のグループは動物実験によるがんの低酸素分布や分子標的療法・放射線治療の効果について評価を行い、これらをまとめるとともにヒトへの橋渡しデータとしても活用できるよう構築した。
FMISO-PETを行った患者群の経過観察については、現在の放射線化学療法の精度が非常に高く、今回の期間において死亡例は見られず、再発の有無に関しても統計学的な有意差は得られなかった。どこかで機会を設け腫瘍内低酸素が予後にどの程度の強度で影響を与えるのかさらなる探求を求めていきたい。
