Research Abstract

もやもや病に対する新規治療法の確立を目的に、ラット脳虚血モデルに対する衝撃波を用いた血管新生誘導の基礎的検討ならびにもやもや病患者に対する血行再建術後の脳血流・血管新生誘導に関する臨床的検討を行った。現在確立されている直接間接血行再建術について連続86例121半球側に対する手術について術後のSPECTによる脳循環動態とMRAによる血管新生を評価し、直接血行再建術の合併症としての過灌流病態(21.5%)、ならびにと成人例における間接血行再建術による血管新生の限界を示唆する結果を報告した(Fujimura et al.Neurosurgery 2011)。以上より間接血行再建術からの血管新生を確実に誘導する手法の開発の必要性が示唆された。基礎実験においてはMatsudaらによるラット慢性脳虚血・間接血行再建術モデルに対して1MPa,5MPa,50MPaの各過剰圧の衝撃波の照射を行い2週間後にパラフィン切片を作製し血管新生の誘導の程度について検証した。結果としては血管新生を認めた個体も多く見られたものの各過剰圧間で血管新生の程度に差を認めなかった。原因としては今回の過剰圧では脳損傷ならびに筋組織損傷を来たした例が見られたことが考えられた。今後は0.5MPaとそれ以下の過剰圧照射による実験を継続する予定である。虚血性心疾患に対しては微弱衝撃波照射による血管新生療法が試みられており、慢性虚血心筋に衝撃波照射を行うことにより血管新生因子が誘導され心機能が改善することが報告されている。脳においては衝撃波の脳組織損傷閾値が1MPa以下と低いため、より精密な照射圧設定と低い過剰圧での検討を要するものと考えられる。
