Outline of Research at the Start |
関節リウマチ(RA)では近年発症の高齢化が言われ、濾胞性ヘルパーT(Tfh)細胞の増加が指摘されている。Tfhの機能制御には濾胞性制御性T(Tfr)細胞が重要であるとされ、RA患者末梢血で Tfh/Tfr比が疾患活動性やACPA抗体価と関連することが報告されている。本研究では、Tfrの機能や疲弊が疾患発症と関連しているという仮説をたて、モデル及びヒトでのTfrの詳細な解析を行う。これらは、RAの発症機序に基づき、病的なT細胞-B細胞の制御を目指し、自己抗体の”量と質“の両者を調整できる発症の根本を断ち切るメカニズムを解析できると考えられる。

Outline of Annual Research Achievements |
関節リウマチ(RA)においては、近年高齢発症者が増加している。RAにおいてCD4陽性T細胞の重要性は明らかであるが、我々は濾胞性Th17細胞がRAの病態増悪に関わり(Ann Rheum Dis 2019)、RORgt陽性制御性T細胞がRA疾患活動性とともに変化する(Rheumatology 2023)ことを明らかにしてきた。また、RAでは制御性T細胞(Treg)の機能低下が報告され、TregのeQTL効果が高く疾患活動性と相関する(Ota M et al. Cell 2021)。我々はRA患者で発症年齢によりTreg細胞が増加していることを見出しており、GPI誘導関節炎モデルでも高齢マウスで関節炎が増悪しTregが増加していた。また、高齢マウスにおいて、Treg細胞の割合が増加し、若年マウスではTregを欠損させることで関節炎が増悪するが、高齢マウスでは影響を受けず、Tregの抑制機能自体が低下していた。本研究では高齢発症RA病態におけるTregの加齢に伴う機能変化およびその原因を明らかにすること、及びその制御の回復に至る機構を明らかにすることを目的とし、今後もRA患者サンプル及び動物モデルを用いて研究を進める。マウスにおいては、関節炎極期のday14において鼠径リンパ節からCD4+T細胞を採取し、single cell RNA-seq (scRNA-seq)により若齢および高齢のGIAにおいて特徴的なクラスター変化および発現変動遺伝子(Differentially Expressed Genes ; DEG)を検出しており、TregクラスターにおけるDEGを抽出し、高齢GIA群で特徴的な遺伝子発現変化を明らかにする。

ヒト、マウス両者の解析がある程度順調に進んできており、scRNSseqのデータも現在展開中で、令和5年度中の論文作成が可能であると考えられる。

Strategy for Future Research Activity |
高齢RA患者、及び高齢誘導関節炎モデルにおいて、T細胞の重要性が顕在化しており、特に制御性T細胞や、その細胞により制御される濾胞性T細胞を絡めて、注目される分子や経路を導き出し、in vivo, in vitro, in silicoで研究を展開していく予定である。
