Outline of Research at the Start

あらゆる心臓疾患の終末像である心不全は世界中で多くの患者の命を奪っている。高血圧・心筋梗塞・心筋症・弁膜症など様々な病態の心臓疾患は最終的に心不全を発症するが、これまで長らくその機序は明らかでなかった。我々は心臓シングルセル解析によって、心筋細胞におけるDNA損傷およびその後のp53シグナル活性化が心不全の原因であることを突き止めた。そこで本研究は、(1)DNA損傷がなぜ生じるか?(2)DNA損傷・p53シグナルは心筋細胞をいかに異常に導くか?(3)異常になった細胞がいかに心不全を誘導するか?(4)これらの知見を基に心不全患者さんの病態を層別化できるか?、を解明することを目指している。

Outline of Annual Research Achievements

我々は2023年度に以下のような実績を挙げた。(1)心筋細胞でDNA損傷が生じる機序の解明:心筋DNA損傷を制御する因子として、NOX4を介したミトコンドリアROS、ビタミンD受容体シグナル(Ito, Komuro et al. Circulation. in revision)、核ラミナ機能(Yamada, Komuro et al. Science Adv. 2023)、CSRP3遺伝子を介したメカノセンシング機構が同定された。さらに、DNA損傷関連遺伝子を網羅的に同定し、シングルセル解析により分子機序を解析している。(2)DNA損傷・p53シグナルが心筋細胞機能を破綻させる機序の解明:心筋細胞の老化が生じる過程で、DNA損傷修復関連遺伝子の発現低下と NOX4を介したミトコンドリアROSの両者が生じることがわかり、それに伴ってDNA損傷の蓄積およびp53シグナルの活性化が誘導された。またDNA損傷の蓄積をキャンセルする転写因子の同定に成功し、その分子機序がほぼ解明された。(3)老化・不全化心筋が心不全を誘導する機序と心筋健常化リプログラミング機序の解明:心筋梗塞巣の心筋細胞における老化が心不全を増悪させる機序や、梗塞境界部におけるメカノセンシング機序を解明した。さらに当初の計画を超えて、DNA損傷と老化を結びつける老化関連分子を標的としたワクチン治療法を開発した(Katoh, Komuro et al. Circulation. in press)。また非心筋細胞のDNA損傷・老化も心不全病態を促進することを解明した。(4)心筋DNA損傷の発症機序・老化心筋の誘導機序:心臓シングルセルRNA-seq解析、組織空間的シングルセル解析、血液プロテオーム解析のマルチオミックス統合解析によって検証し、空間的オミックス解析によって心不全病態の層別化アルゴリズムを開発している。

Reason

当初に計画していた研究はほぼ遂行済みであり、当初の計画を超えて、DNA損傷と老化を結びつける老化関連分子を標的としたワクチン治療法を開発し、論文受理まで進めることができた。また非心筋細胞(線維芽細胞・内皮細胞)のDNA損傷・老化も心不全病態を促進することを明らかにすることができた(2本の論文がいずれも論文リバイズ中)。

Strategy for Future Research Activity

(1)心筋細胞でDNA損傷が生じる機序の解明:CRISPRi iPS心筋を用いてDNA損傷関連遺伝子の機能を網羅的に解析し、心筋細胞においてDNA損傷を制御する重要な分子を複数同定することに成功した。その分子機序の解明を進める。(2)DNA損傷・p53シグナルが心筋細胞機能を破綻させる機序の解明:DNA損傷・p53シグナルが転写制御に与える影響をCUT&RUNなどのエピゲノム解析し、さらにはシングルセルレベルでクロマチン高次構造や遺伝子発現の変化を解析する。(3)老化・不全化心筋が心不全を誘導する機序と心筋健常化リプログラミング機序の解明:心筋健常化リプログラミングに関する機序として各種転写因子の機序を解明する。また運動療法が心筋細胞の健常化を誘導する機序も明らかにしたため、その分子機序も合わせて明らかにする。(4)心筋DNA損傷の発症機序・老化心筋の誘導機序:さまざまなプラットフォームの空間シングルセルオミックス解析を構築したため、これによって心不全病態の層別化アルゴリズムを構築する。
