現在の日本の精神医療は、精神障害当事者の意思と価値に基づく支援・治療・研究開発の意義が認められつつあるが、その実現にとって多くの障壁が残されている。数あるこうした障壁のうち、本研究は精神医療にそなわる認識方法である病識評価に着目し、これが精神障害の経験を理解し表現するための概念的資源を構造化し、制約してきた状況を解明する。解明の対象には、戦後から現在までの日本の主要精神医学雑誌記事と当事者による手記を設定し、その叙述における関連概念の用法を分析する。これを通じて本研究は、精神障害の経験を理解し表現するための既存の概念的資源にそなわる倫理的問題を明らかにし、その改良に向けた提言を行う。

| Outline of Final Research Achievements |
This study elucidates the forms of epistemic injustice imposed on persons with mental disabilities through the concepts of insight and lack of insight, employing an ethnomethodological conceptual analysis. Specifically, it examines (1) how terms related to insight and lack of insight are used in memoirs written by persons with mental disabilities and in commentaries on those texts and (2) how support practices targeting people with mental disabilities respond to problems of epistemic injustice related to insight. Through this analysis, the study clarifies how the concept of insight is articulated in both personal narratives and support practices and how it contributes to the construction of epistemic injustice.

| Academic Significance and Societal Importance of the Research Achievements |
哲学者M.フリッカーは、マイノリティのこうむる認識と表現にかかわる概念的資源の不正なあり様を「認識的不正義」と呼んだ。病識概念のもとで精神障害当事者が強いられてきた状況もそのひとつである。本研究はこの状況を問題と捉え、病識・病識欠如という精神医学的概念を社会的コンテクストのなかに置き直して検討した。具体的には、精神障害の当事者による手記とそれらについての論評、精神障害者を対象とする支援実践を対象に、関連諸概念の用法の分析的解明を通じて病識概念にまつわる認識的不正義のあり様を具体的に特定した。そのうえで、この問題に志向して行われている支援実践を主題に、その相互行為的組織方法を記述的に解明した。
