Outline of Research at the Start

注意欠如・多動症(ADHD)は不注意・多動-衝動性を主徴とする神経発達障害である。ADHDは多様性を持つにも関わらず、臨床症状を基に一つの疾患としてまとめて扱われてきたことで、研究結果に一貫性がなく、未だにその神経基盤は十分にわかっていない。本研究の目的は、安静時fMRIによって検出される神経ネットワークに基づき、機械学習を用いてADHDのサブタイプを特定するのと共に、サブタイプごとの神経基盤と臨床的な特徴を明らかにすることである。本研究はADHDの病態解明につながるのみではなく、非侵襲的かつ客観的なADHDのサブタイプ診断法と個々の患者の病態に応じた根本的な治療の開発に貢献するものである。

Outline of Annual Research Achievements

米国における、約12000名が参加する大規模な縦断的多施設共同研究ABCD (Adolescent Brain Cognitive Development) Studyのデータベースを活用して、教師な し機械学習によるADHDのサブタイプ研究に取り組んだ。 データをクリーニングし、ADHD群656名、nonADHD群6601名を抽出した。ADHD群に対して、認知機能検査のデータに基づいて階層的クラスタリングを行い、3つの クラスターに分類した。3つのクラスターは全体的な認知機能が高いAタイプ（254例）、処理速度が特に低いBタイプ（212例）、処理速度以外全体的に認知機能が低いCタイプ（190例）に分類された。さらに、各ADHDサブタイプとnonADHD群とで脳構造容積を比較したところ、ADHDのCタイプのみ、右外側眼窩前頭皮質、左下側頭回において脳容積の減少を認めた一方で、ADHDのAタイプとBタイプには有意差を認めなかった。この成果をまとめ、現在論文を投稿中である。

また、同様にABCD Studyのデータを活用し、妊娠中の大麻使用が児の認知及び脳発達のプロセスに与える影響について検討した。その結果、妊娠中の大麻暴露群は、非暴露群に比べて、視知覚に関わる認知機能と頭蓋内容積の変化が小さいことが明らかとなった。この研究成果は2023年 に国際科学雑誌Developmental Cognitive Neuroscienceに掲載された。

さらに、現在、睡眠クロノタイプと脳構造の関連、スクリーンタイムと脳構造の関連についても成果をまとめ、論文投稿中である。
