| Outline of Research at the Start |
粒子状物質による健康被害について最近関心が高まっている。黄砂やPM2.5は大きさや構成成分などが異なるため、影響を受ける病態は最終的には多種多様になることが推測される。本研究の目的は、黄砂やPM2.5といった粒子状物質が重篤な病態である急性心筋梗塞や心停止発症にどの程度影響するのか、またその臨界値を推定できるのかを調査することである。心停止の原因は心原性のみならず、脳血管疾患や呼吸器疾患、悪性新生物など多岐にわたるため、粒子状物質による疾患感受性や粒子状物質の化学成分に基づいた発症メカニズムについて検討を行うことで、黄砂やPM2.5に対する臨床的予防や管理の意義について新たな知見を築く。

| Outline of Final Research Achievements |
This study conducted an epidemiological investigation into the effects of short-term exposure to PM2.5, a particulate matter, on disease onset and mortality. An increase in PM2.5 concentration has been found to have a significant impact on the incidence rates of respiratory-related out-of-hospital cardiac arrest and acute myocardial infarction (AMI), with black carbon components contributing notably to this effect. Furthermore, the risk of AMI increased significantly following the 2016 Kumamoto earthquake, indicating that changes in living environments caused by natural disasters may influence the risk of cardiovascular diseases. In addition, black carbon, a component of PM2.5, has been linked to the onset of AMI. However, no such associations were observed with water-soluble organic compounds, nitrate ions, or sulfate ions. These findings provide clarity regarding the health effects of particulate matter concentrations and their component-specific impacts.

| Academic Significance and Societal Importance of the Research Achievements |
粒子状物質による短期曝露が循環器疾患や呼吸器疾患のリスクを有意に高めることを疫学的に実証し、特に黒色炭素成分が重要な寄与因子であることを明らかにした点で学術的意義が高い。また、自然災害による環境変化が循環器リスクに与える影響も示し、環境医学、公衆衛生学、循環器病学の分野に新たな視点を提供した。さらに、粒子状物質の成分別リスク評価という新たなアプローチを提示した点でも意義が大きい。社会的には、大気質改善や災害対応を含む総合的な健康リスク管理の重要性を明確にし、循環器疾患予防、環境政策、災害医療体制整備に資する基盤情報を提供した。今後、地域特性や高リスク集団に応じた精緻な対策立案が求められる。
