Outline of Research at the Start

我々は,脳卒中発症前リスクを可視化することを目的に,医療技術,大規模コホート研究,工学技術の3つを融合させることにより,血管走行・壁ストレス標準モデルを作成する我々の手法:MRAをMontreal Neurological Institute (MNI)templateにより標準座標へ変換することにより血管走行位置座標を標準化し,さらに血管の中心点を抽出し分岐点を同定することにより,標準座標上の血管走行を同定することは独創である.

Outline of Annual Research Achievements

血管走行を標準化するため,MRA画像をMNI座標に変換する方法を複数検討中である.本研究では各断面で血管の中心点を抽出し,中心点を結ぶことにより中心線を求め,中心線を平均化して標準血管走行を構築する.脳血管は,椎骨・脳底動脈系,内頸動脈,前大脳動脈,中大脳動脈,後大脳動脈に大きく分類され,中心線の分岐点を抽出することにより個々の血管の起始点,終止点を同定する.我々はすでに,高血圧患者40例と正常例40例において,標準血管を作成し脳底動脈を抽出して座標比較をし,脳底動脈起始点は,高血圧患者で正常例に比べ有意に上昇していることを確認した. 本研究では,東北メディカル・メガバンク機構の大規模コホート症例を用い,血管走行・壁ストレス標準モデルに対して,個人の逸脱の程度を評価する方法として,血管の位置座標のmeanとSDを用いることを考えている.脳底動脈,内頸動脈,中大脳動脈,前大脳動脈の標準位置情報にmeanとSDが求められる.東北大学病院では,すでに原発性アルドステロン症による高血圧症例のMRA画像を有しており,原発性アルドステロン症と正常例の鑑別を目的として,血管走行・壁ストレス標準モデルからの逸脱の定量パラメータを決める.壁ストレス測定のためにComputational Fluid Dynamics解析を行う.我々は血液濃度,粘度と脳底動脈流速を文献より理論値として用い,脳底動脈の面積が狭いところでは血管ストレスが高いことを確認した.本研究では,個別のinput functionとして内頸動脈,椎骨動脈流速を用いてCFD解析を行う.壁ストレスは,血管壁の各pixelの壁ストレスのヒストグラム解析によるパラメータや,壁ストレス不均一性を定量化する.日本人正常例各年代の血管走行標準モデルに対して,血管壁ストレスマッピングを行い,血管走行・壁ストレス標準モデルを作成する予定である.

Strategy for Future Research Activity

今後,確立した標準化方法と中心線抽出方法をスーパーコンピュータ内に移行しコホート症例での標準血管走行を座標として算出する予定である.
