| Outline of Research at the Start |
治療効果向上と有害事象軽減を目指した「個別化放射線治療」を実践する。
局所進行悪性腫瘍に対し、末梢リンパ球や血中の腫瘍由来エクソソームに存在する、mRNAやmiRNAの発現パターンを調べ、放射線治療効果を予測する。またDNA修復能の指標として末梢リンパ球の放射線誘発ガンマH2AXフォーカスを測定する事で有害事象を予測する。これらの手法を用いて個々の患者毎の特性に合わせて線量分割や照射体積を調整する。

| Outline of Final Research Achievements |
We analyzed radiation-induced gamma-H2AX foci in peripheral blood lymphocytes and tumor-derived exosomal miRNA expressions in plasma from 45 cervical cancer patients who underwent definitive radiotherapy, The results showed that the decay ratio of gamma-H2AX foci can predict the occurrence of late adverse events, and that a risk score combining nine exosome-derived miRNAs signatures in plasma can predict the treatment outcome.

| Academic Significance and Societal Importance of the Research Achievements |
本研究によって、子宮頸癌に対する根治的放射線治療後、半年から数年の間に起こる晩期有害事象（副作用）の予測が、治療開始前の血液を調べる事で可能となる可能性を示す事ができた。また、局所再発や遠隔転移といったリスクの高い患者を治療前に見つけ出す事で治療効果予測が可能となった。これを他癌腫にも進める事によって、個人毎のより最適な放射線治療スケジュールが決定でき、個別化放射線治療を実現する足掛かりとなり、将来的に癌治療の全体的な成績向上に寄与が期待できる成果を得る事ができた。
