Outline of Research at the Start

近年、提案されている新技術に、申請者固有技術のdeep learningを組み込んだ臓器segmentationを組み合わせ、肝転移に対するEOB-MRI撮影及び診断を包括的に改善する事を目的とする。本研究の特徴として、EOB-MRIに関連した新規技術の臨床導入を明確に意識している点を挙げる。実臨床への波及効果の視点から先行研究の意義を再定義し、有機的に結びつける事を目指す。

Outline of Annual Research Achievements

昨年の成果を基に、画質向上と診断能の向上に向けたさらなる取り組みを行った。
拡散強調画像のプロトコル改善において、SNRの安定性と微細病変の見落としに対処するために、2回息止めのプロトコルを運用した。しかし、症例によっては息止め位置の不安定さに起因する病変の見落としを経験したため、呼吸同期のDWIシークエンスに変更を行った。
息止めのsingle-shot T2WIは深層学習によりノイズが低減する一方、ETLが延長する事に伴うblurringが顕在化して見える様になった。診断能向上のために、同blurringの解消に取り組んだ。具体的には、3D収取で用いられているvariable flip angleを2D収集にも適用する事で、[1] T2 blurringの解消によるコントラストの改善 [2] black-blood効果に伴う脈管の信号の抑制を行う事で、脈管近傍の信号変化の描出能の改善 [3] refocus flip angleを下げる事によりエコースペースを低下させ、更なるT2 blurringの解消と撮像時間の短縮を達成した。
クラウドベースで3D U-netをベースとした深層学習パッケージのセットアップを行った。非M.D.である研究補助員のトレーニングを行い、CTを用いた確立されたsegmentation法をfusionしたMRI画像にコピーする事で、医師のeffortを最低限にしつつ、MRI-segmentationの教師データを作成するworkflowを確立した。初期検討を行った所、DICEで80%前後程度の結果を達成可能であった。今後、症例数の蓄積、正解データのブラッシュアップ、3D U-netの適応前の信号値の正規化等について、更なる検討を行う。

Reason

深層学習再構成によりT1WIやT2WIの画質向上は得られたものの、3D収集シークエンスや拡散強調画像の画質向上が得られなかった。
新規のパッケージのインストールを行う事で解決が可能と考えられた。

Strategy for Future Research Activity

2024年4月より、3D収集やradial samplingのDWIにも対応可能な深層学習アルゴリズムを導入した。
dynamic MRI及び肝細胞造影相MRIの画質が著明に向上する事を初期検討で確認した。
今後、DWIプロトコルの更なる作り込みを行い、研究を遂行する。
