Outline of Research at the Start

前立腺癌に対する放射線治療成績を十分に検討できるデータ構築と統計学的手法を用いた様々な解析により,従来の70 Gy/35 fr以下の線量で得られた知見を前提とした放射線治療を高精度治療時代に合致するがん治療に革新することを目的として,エビデンスに基づく臨床試験の実施に繋げることを目的とする.同時に,リスク臓器のDVH解析に基づいた個別化されたリスク臓器の耐容線量,分割回数の変化による線質別の生物学的効果比を算出する.最終的には,日本人に対する照射技術別の標準治療を確立し,全国に普及させる.さらに,利便性や経済的な面で有望な寡分割/超寡分割照射法の有効性を検証する新規臨床試験を企画し実施する.

Outline of Annual Research Achievements

陽子線治療では、寡分割照射(70Gy/28Fr)のII相試験の100例について解析し、Grade 2
の消化器系、尿路系有害事象は、それぞれ1%、4%であった。さらに短期化した治療(63Gy/21回)のII相臨床試験が全例観察期間を終了したため、現在データクリーニング中である。
IMRTでも同様の70Gy/28Frの線量分割によるII相試験を134例に実施し、5年非再発率は96%、Grade 2の消化器系、尿路系有害事象は、ともに5.3%であった。
定位放射線治療では有害事象について線量分布を解析した。急性期有害事象は前立腺体積と中等度の線量が入る直腸体積、慢性期有害事象は膀胱・直腸の最大線量と相関がみられた。また、定位照射におけるサイバーナイフとVMATを比較し、急性期尿路症状が前者で有利な可能性が示唆された。また、サイバーナイフによる前立腺定位照射の線量増加試験を行い、35Gy/5回のほうが37.5, 40 Gy/5回より有害事象が少なかった。
重粒子線治療では、マーカー留置時の適切なマージン設定のために、15例に対して治療中のマーカーの動きを検討した。その結果、95%タイル値で腹側方向への異動が2㎜を超えたがその他の方向は2mm以内であった。骨照合のみの標的に対するV95%が98.2%であったのに対し、マーカー使用時は99.5%であり良好であった。また、高リスク前立腺癌の治療成績を予測する特有の新たなノモグラムを作成し、論文を投稿中である。
前立腺癌の患者説明用動画を作成し、QSTチャンネルで公開し、複数の放射線治療施設にURLを送付した。今後、JROSG-HPにリンクする予定である。

国際誌として初年度6編、2年目に8編、本年度も8編の論文を公表できた。また、説明用動画の作成が終了し公開した。
一方で、投稿中の論文が1編、投稿準備中の論文2編あり、解析追加と予算確保のために来年度まで1年間研究の延長申請を実施した。

Strategy for Future Research Activity

引き続き、JROSG泌尿器腫瘍班、研究分担者との意見交換の場を確保し、積極的な検討を実施する。定位照射、重粒子線治療の超寡分割照射については患者のリクルートをJROSG,およびJ-CROS等で依頼する。
