| Outline of Research at the Start |
小児がん治療において、切除不能な固形がんに陽子線治療が用いられるようになり、有害反応のリスク軽減が期待されているが、解剖学的に腫瘍と消化管が近接する症例では根治治療は陽子線治療を用いても不可能である。我々は腫瘍と近接臓器とに間隙を作るPGA(ポリグルコール酸縫合糸)を用いた吸収性スペーサー(ネスキープ)を開発し、小児領域でPhase I 臨床試験を実施している。しかしながら、本治療法を小児がんに適応する際には、化学療法併用の影響など更なる検討が必要である。本研究では小児がんの治療を想定したラットモデルを構築し、化学療法併用時の生体への影響、周囲の微小環境について検証する。

| Outline of Final Research Achievements |
We created a rat model to evaluate the effects of inserting PGA spacers with cyclophosphamide (CPA). We compared the chemotherapy-free group with the chemotherapy group. We implanted spacers in the rats and used CT imaging to evaluate the administration of CPA. We quantified morphological changes, spacer size, and volume; then, we evaluated adhesion and tissue histology using pathological specimens. Macrophages were observed around the PGA fibers, suggesting their potential involvement in PGA spacer degradation. No significant differences were observed between the two groups. These results suggest that CPA does not cause clinically significant degradation or harmful tissue reactions with PGA insertion.

| Academic Significance and Societal Importance of the Research Achievements |
小児腹部腫瘍の放射線療法において、同時化学療法はしばしば行われる。また近年、陽子線治療時に正常組織と腫瘍の間隙を保つために、ポリグリコール酸(PGA)製の体内で吸収されるスペーサーが開発され、臨床的に用いられているが、化学療法がスペーサーに与える影響は検証されていない。ラットモデルを用いて、シクロホスファミド(CPA)がPGAスペーサーに与える影響について評価を行い、CPA投与群と非投与群において、で有意な差は認められなかった。これらの結果により、化学療法を行った場合も臨床的に問題となるようなPGAスペーサーの吸収速度の変化や有害な組織反応を引き起こさないことが示された。
