Outline of Research at the Start

悪性リンパ腫を含む造血器腫瘍では細胞死誘導性の p53シグナルが保たれており、p53の再活性化を惹起する免疫化学療法や放射線治療に比較的感受性が高い。最近、リンパ腫特異的な異常シグナルを標的にした、理論的には p53非依存的に薬効を示す分子標的治療が利用されるようになったが、実地診療ではしばしば p53異常クローンの拡大が認められる。本研究では、長期の分子標的治療によって、p53シグナル修飾と機能的ヒエラルキー変化による分子標的治療耐性機序が形成されるが、適切な分子標的治療との併用や周期的治療などの工夫によって、薬剤耐性は打破できるという仮説を検証する。

Outline of Annual Research Achievements

造血器腫瘍は免疫化学療法や放射線治療に感受性が高いが、これら治療は腫瘍細胞のp53の再活性化と、その下流の細胞死誘導性p53シグナルの活性化を惹起する。造血器腫瘍ではp53シグナルの活性化が細胞死をきたしやすく、通常抗がん剤や放射線治療感受性が高い。このようにp53シグナルは造血器腫瘍治療のキーとなるシグナルではあるが、それのみでは、少なくとも一部の患者では治癒をもたらすことができないことも明らかになっている。最近、造血器腫瘍では異常シグナルを標的にした p53非依存的に薬効を示す分子標的治療が利用されるようになっている。それにも関わらず、実地診療では耐性化とともに p53異常クローンの拡大が認められる。本研究では、分子標的治療によるp53シグナルとの干渉、そしてp53シグナル修飾と機能的ヒエラルキー変化による分子標的治療耐性機序を解明することを目指した研究において、BCL2阻害耐性の新規耐性因子としてのMYCシグナル異常と、そして p53活性化を惹起する分子標的薬が p53シグナル依存性にMYCシグナルを抑制することを見いだした。研究成果は Sience Advancesに報告した。

Strategy for Future Research Activity

p53シグナル依存性の細胞死誘導は、p21発現集団では低下し耐性化をきたしうるが、このポピュレーションはBCL2阻害に感受性が高いことを見いだしており、治療学上の相補性を有することがわかった。p53のマスターレギュレーターであるMDM2に関して、MDM2-p53結合の阻害は臨床試験を含めてp53ダブルヒット変異では無効であったが、MDM2の分解促進は部分的にp73シグナルを介した腫瘍細胞死を誘導することが報告されており、今後MDM2-p73/p53シグナルを含めた研究を進めてゆく。
