Outline of Research at the Start

Outline of Final Research Achievements
To elucidate the role of PD-L1 containing exosomes in sepsis, we investigated immune dysregulation in post-cardiac arrest syndrome (PCAS). Plasma levels of soluble PD-L1 (sPD-L1) were measured during cardiopulmonary resuscitation (CPR) in 30 out-of-hospital cardiac arrest patients. sPD-L1 levels were markedly elevated, comparable to those in sepsis, and showed significant correlations with decreased peripheral lymphocyte counts and elevated C-reactive protein levels. In seven patients who achieved return of spontaneous circulation (ROSC), sPD-L1 levels increased in parallel with organ dysfunction. These findings suggest that ischemia-reperfusion during CPR may aberrantly activate immune and endothelial cells, leading to excessive sPD-L1 release. This mechanism may play a pivotal role in immune exhaustion and organ failure in PCAS and offers novel insights into immune paralysis in sepsis.

Academic Significance and Societal Importance of the Research Achievements
