Outline of Research at the Start

進行・再発卵巣粘液性癌は化学療法抵抗性を示し、難治性癌である。病理組織診断の精度が低く、稀少性もありゲノム解析に基づく十分な抗腫瘍効果を示す治療法は確立していない。
本研究では、粘液性癌、発生母地である境界悪性腫瘍や嚢胞性腺腫、鑑別診断で重要となる転移性粘液性癌、他臓器粘液性癌(胃癌、小腸癌、結腸癌、膵癌、子宮頸癌)の検体を用いたオミックス解析を行い、①原発性粘液性癌の病理診断精度向上に寄与するオミックス解析結果を検討し、②RNA解析による1細胞発現解析により難治性卵巣粘液性癌の治療抵抗性に関連する免疫微小環境における分子機構を解明し、難治性卵巣粘液性癌に対する新たな治療戦略を開発する。

Outline of Final Research Achievements

A plasma metabolome analysis of patients with gynecological cancer, including refractory ovarian mucinous carcinoma, revealed significant increases or decreases in metabolites such as amino acids and phospholipids in plasma compared to a general population cohort, confirming that multiple metabolites related to diagnosis, prognosis, and treatment efficacy fluctuate, indicating the potential for the search for biomarkers useful for predictive medicine. We are conducting an integrated analysis that aims to explore treatment targets for refractory gynecological cancers by adding plasma proteome analysis.

Academic Significance and Societal Importance of the Research Achievements

利便性の高い血液検体(血漿)を用いた絶対定量可能なメタボローム解析により、難治性希少癌を含む婦人科癌の早期診断、予後・治療効果予測を可能とする多層的オミックス解析の有用性を示した本研究成果により難治性婦人科癌に対する治療の精緻化が期待される。プロテオーム解析を含む多層的オミックス解析により抽出された標的に着眼したReverse translational researchにより卵巣粘液性癌をはじめとする難治性婦人科癌の治療開発が期待される。
