Outline of Research at the Start

加齢に伴う難聴は内耳細胞の老化と共に不可逆的に進行する。ゆえに、その発症メカニズムの解明と予防戦略の確立は超高齢化社会を迎えた我が国にとって近々の課題である。申請者の研究グループは加齢に伴って、早期に難聴が発症・進行する遺伝性難聴の一つとして、ファブリー病の存在を発見した。
本研究ではファブリー病モデル動物を用いて、「遺伝子変異による代謝異常産物の蓄積が、内耳の細胞老化を促進させて、早期に難聴を発症・進行させる」という仮説を検証する。
この研究成果は、複雑なメカニズムで発症すると考えられている「加齢に伴う難聴の発症の予防と進行を制御する治療戦略モデルの一つとなる」ことが期待できる。

Outline of Final Research Achievements

We conducted an analysis of physiological and biochemical clinical laboratory data as well as subjective symptoms in patients with Fabry disease, characterized by high-frequency hearing loss with onset in childhood and progressive deterioration with age. Our findings revealed that certain younger patients reported pronounced subjective symptoms of hearing loss and vertigo, which were not commensurate with their physiological test results. Moreover, sudden sensorineural hearing loss was observed in 38% of the patients included in this study.
In addition, to investigate age-related changes in cochlear blood flow, we performed intraoperative observations of the auditory system in human subjects. Through this approach, we established a method for capturing video recordings of the superficial vasculature of the tympanic membrane and analyzing blood flow velocity using dedicated image analysis software.

Academic Significance and Societal Importance of the Research Achievements

ファブリー病患者の聴力は、加齢に伴う難聴の他に急性感音難聴を高率に引き起こし、純音聴力検査と比較して難聴の自覚症状が強いという特徴が考えられた。また、加齢に伴う難聴と聴覚器の血流がどのように関係しているか解明することを目指し、内視鏡動画を用いて鼓膜表層の血流速度を測定する方法を世界で初めて構築した。症例の集積や他の部位への応用によって、加齢と聴覚器における血流速度との関係の解明が期待される。
