本研究は、高度実践看護師の患者との対話場面における「沈黙」について、看護師はどのように解釈しているのか、また患者の表現としての沈黙をどのように推測しているのか、さらに意図的沈黙をどのように活用しているのか、その「沈黙の意味解釈」と「活用技法」を明らかにすることを目的とする。
具体的には、①国内外の文献から対話場面における沈黙の意味や相互作用に関する資料を収集・整理し概念枠組みを構築する。②高度実践看護師を対象に患者との対話場面における沈黙の意図や体験、意味・解釈、活用技法についてインタビュー調査を行う。③語りを質的帰納的に分析し、概念モデルの作成と構造化を図る。

| Outline of Final Research Achievements |
A final seven advanced practice nurses were interviewed. In addition to the four core categories of “interpretation of the meaning of silence,” “techniques for utilizing silence,” “original experience of silence,” and “educational use of silence,” the “complexities of silence” were identified. The new “Complexity of Silence” was defined by the patient's background, the patient-nurse relationship including interaction, and the environment such as time and space together. During the silences, the nurse was simultaneously observing the patient's facial expressions, eye contact, and other aspects of the patient's behavior and reaction to the nursing action, constant assessment and reflection on patient understanding and interpretation of the meaning of the silent phenomenon, active waiting and judging when to break the silence, and professional nursing actions including deliberate and subtle gestures. The silence was developing and progressing simultaneously.

| Academic Significance and Societal Importance of the Research Achievements |
本研究により、高度実践看護師の患者との看護場面における沈黙の意味解釈と活用技法を明らかにすることができた。高度実践看護師側の解釈ではあるものの、患者の沈黙の理由についての解釈の幅は、基礎教育や現任教育における推測力や判断力の礎として、また患者理解の深化の一助となり、教育的に活用することができる。活用技法についても、患者背景、関係性など多種多様に状況依存的ではあるもの、状況判断や介入など看護師の実践力の向上育成にも寄与できるものである。これらにより沈黙を活用した看護力が向上することは、患者の意思決定支援や療養生活の向上につながるものと考える。
