| Outline of Research at the Start |
地域包括ケアを進める中で特定行為に係る看護師（以下、特定看護師と称す）の養成が進められているが、在宅での特定看護師は依然として増えず、特定看護師研修受講済みの看護師も特定行為を十分に実践できていない現状がある。そこで、本研究では、在宅で活動している特定看護師による①特定行為実践の現状と効果および実践を妨げている多面的な要因を明らかにし、②問題解決に向けた在宅実践モデルを構築することを目的とする。本研究は、在宅における特定行為への理解を進め、訪問看護師の特定行為研修受講を促進し、特定行為を行う上での課題と方策を提示するものであり、今後の訪問看護や在宅医療の発展において意義がある。

| Outline of Final Research Achievements |
An interview survey clarified the factors that do not lead to the practice of specified medical acts nurses in the home (inhibiting factors) and the factors that lead to it (facilitating factors). Inhibiting factors included a lack of benefits for doctors, "Doctors are not aware of the specified medical practice training system," "There is no need in the community," and "There is no relationship with doctors."
To find out how doctors perceive this, a questionnaire survey was conducted (mailed to 3,600 doctors nationwide, with 597 responses). Only 47% of doctors were aware of specified medical acts nurses, and only 11% had actually requested their services. However, there was a positive attitude, with 82% believing that " specified medical acts nurses can reduce emergency visits by doctors," 87% believing that " specified medical acts nurses can reduce the burden on doctors," and 75% believing that "specified medical acts nurses are necessary.

| Academic Significance and Societal Importance of the Research Achievements |
在宅医療において医師の指示を待たずにタイムリーおよびスピーディーに医療行為ができる特定看護師が必要とされ、養成が2015年より開始された。しかし、活躍できていない特定看護師が多いというインタビュー調査結果があった。そこで特定看護師が実践できない要因を明らかにする必要性があり、医師への質問紙調査で明らかにした。結果、医師の認知度の低さが明らかになったが、特定看護師には好意的で今後の連携や活躍を期待されていた。このことは、特定看護師にとってさらに自信を持って積極的に医師や国民にアピールしていく必要があることが明らかとなった。この結果は学術的・社会的意義があったと言える。
