Outline of Research at the Start |
本研究は、てんかんにおける精神・行動面の合併症(うつ・不安、精神病、発達障害等)につき、脳画像を用いてその脳神経メカニズムの解明を試みる。精神・行動的合併症は有病率が高く、てんかん患者の生活の質に及ぼす影響が大きい一方で、それが生じる脳のメカニズムについては報告が少ない。一般人口における精神・行動障害と同様のメカニズムを持つのか、それとも異なるのかにより、有効な治療法・介入法等についても差異がある可能性があり、これらの解明は治療法の改善点やその後の生活の質改善に寄与することが期待される。本研究では、高解像度脳MRI撮像と最先端の解析技術を用いることで、低侵襲にこれらを明らかにする。

Outline of Annual Research Achievements |
2022年度はまず、てんかんに合併しうる重要な精神症状である精神病症状について、その神経基盤・メカニズム解明のため、発作間欠期の18F-FDG-PET画像およびMRIを解析し、小脳上部から中脳にかけての糖代謝亢進が起こっていることを発見し、国際学会(14th Asian & Oceanian Epilepsy Congress)および国際誌にて発表を行った(Sone D, et al. Epilepsia Open. 2022)。近年、小脳について、運動機能のみならず認知や感情との関連を示唆する報告が相次いでおり、てんかん精神病についても同部位の関わりが示唆された。その後、この結果に加えて、てんかんに合併する精神病症状の神経基盤と脳画像所見に関するこれまでの国内外の既報論文をまとめた総説を発表した(Sone D. Front Psychiatry. 2022)。更に、てんかんや他の精神神経疾患において更なる応用と発展が期待されるバイオマーカーであるNeuroimaging-Based Brain Age Estimationについても、100報以上の論文をまとめた総説を発表した(Sone D & Beheshti I. J Pers Med. 2022)。これらの総説論文により、てんかんの精神症状や脳画像バイオマーカーにおける現状と問題点が更に整理され、今後のこの分野の研究発展に寄与されることが期待される。現在は、拡散テンソル画像を用いた白質繊維に基づく新たなバイオマーカーについて、主に人工知能や機械学習の手法を用いて解析中であり、一部は既に結果が出ており学会発表および投稿準備中である。
