Outline of Research at the Start

脳転移患者の予後向上により、放射線治療後の認知機能低下の低減はより重要な課題となっている。認知機能は記銘力や遂行力/注意力、言語流暢性など様々な要素から構成されており、放射線治療後の認知機能低下にも様々なスペクトラムが含まれている。
放射線治療の方法や照射量の違いによる影響を認知機能の各要素ごとに評価することで、脳転移放射線治療後の認知機能の予測指標を確立する。

Outline of Annual Research Achievements

脳転移放射線治療後の認知機能の予測指標を確立させるために以下の2つの軸で研究を進めている。
①既存の全脳照射後の認知機能検査のデータを使用し解析:2023年度は照射後3年までの解析データをもとに論文及び、国際学会での発表準備をした。
②比較的新しい治療法の前向き観察研究にて認知機能に関わるデータの蓄積:多発脳転移に対するSingle-isocentervolumetric modulated arc therapyや脳転移術後の術後定位放射線照射について治療成績をまとめ発表した。(当院における脳転移切除後の術後腔定位照射の初期経験:中野、第82回日本医学放射線学会総会)、(当院における多発脳転移への定位照射の新たな試み:中野、第46回新潟肺癌研究会総会)認知機能及び健康関連QOLに関する前向き観察研究の準備を進めた。

Strategy for Future Research Activity

①本年度中の論文投稿を目指す。もしくは国際学会での発表を目指す。
②多発脳転移に対するSingle-isocenter volumetric modulated arc therapy(直線加速器にて複数個所の病変に対して同時に定位放射線照射を行う)に関して認知機能検査を前向きに取得する観察研究を立ち上げて症例の集積を行うとともに、認知機能に影響を与えるであろうサイズの大きな病変に対する治療成績についても後方視的に解析をすすめる。
