Outline of Research at the Start

急速な人口の高齢化は日本のみならず、世界各国で問題視されており、手術後に高齢者に多く発症する術後せん妄が注目されている。術後せん妄は、術後認知症および生命予後に関連するため、早急な対策が不可欠であるが、未だ確立した薬物治療法は存在しない。
これまで申請者らは、術後せん妄モデルを開発し、術後せん妄の新規治療薬として、神経ステロイドAllopregnanolone(ALLO)の有効性を検証してきた。
本研究は、神経ステロイドALLO誘導体による新規治療薬の効果を、高齢ラット術後せん妄モデルを用いて、静脈内投与と内服の両方の投与方法で比較検討し、安全性および有効性を検証することを目的とする。

Outline of Annual Research Achievements

術後せん妄は、特に高齢術後患者の認知機能の低下および生命予後に影響し、高額な医療費の増大を招いていることから重大な周術期合併症として注目されている。しかし、薬物による予防・治療法は未だ確立されていない。
本年度も術後せん妄の主要な病態機序と考えられている脳内神経炎症に対して、抗脳内神経炎症作用が期待される神経ステロイドの一種Allopregnanolone (ALLO)の有効性、およびその作用機序を高齢ラット術後せん妄モデルを用いて検討した。術後せん妄モデルに対してALLOの静脈内投与と内服の有効性をRT-PCR法を用いて比較検討した結果、両投与方法の有効性を示した。そこで、臨床現場において投与による負担がより軽度ある内服投与を中心に、ALLOの抗脳内神経炎症作用の新規作用機序の解明を目的としてRNASeq解析を実施した。
当該年度は、RNASeq解析で得られた多くの候補遺伝子の解析を継続して実施した。脳内神経炎症に関連する可能性があり、かつより大きく変化した遺伝子を抽出し、得られた遺伝子の役割を過去の文献を参考に検証し、得られた遺伝子経路についても検討した。これらの解析で候補となる遺伝子の絞り込みを行っている最中であり、その後RT-PCR法にてALLO内服薬の新規作用機序の解明に挑む予定である。
