Outline of Research at the Start

わが国の在宅終末期患者へのAdvance Care Planning (以下、ACP)においては、患者の意思を継続的かつ随時確認できる仕組みの構築が喫緊の課題である。本研究は患者の変化への対応に効果が検証されている電子的患者情報アウトカム (以下、ePRO)をACPに応用する。本研究は在宅医療の現場で患者の治療・療養に対する望みを医療従事者と患者・家族が電子的に情報を共有して話し合いを行う「ePRO型ACPモデル」を開発し、実践的に導入することで新たな仕組みを構築する。

Outline of Annual Research Achievements

2023年度は研究計画に基づき、「研究結果の公表」「改良されたePRO型ACPモデルの継続導入」「ePRO型ACPモデルの検証」を行った。
研究結果の公表では、パイロット研究のにおいて明らかになった成果を国内および国外において学会発表を行った。プロトコール作成の基盤となったデータにおいては論文としてまとめており、投稿した論文は現在査読中である。
改良されたePRO型ACPモデルの継続導入では、昨年度に引き続きパイロット調査を経て作成したモデルを株式会社インテグリティ・ヘルスケアが提供するオンライン診療プログラムであるYaDocへ実装し、6つの往診クリニックで実践的に導入した。本研究の対象者スクリーニングはサプライズクエスチョンを用いて実施した。対象者には12カ月にわたりモデルを継続して導入し、2023年度内においてすべての対象者における継続導入を完了した。
ePRO型ACPモデルの検証では、介入群(モデルの導入)と対照群(従来通りの対応)を無作為に2群に分け、それぞれの患者と対応にあたった医療者を対象としてアンケート調査を行った。調査は1カ月・3か月・6か月・12カ月の4つのタイミングで縦断的に実施した。これらのデータはすでに収集を終えており、今後は解析を予定している。さらに、対象者が調査期間中に死亡した場合、その時点から3か月後に遺族調査を実施しており、これらのデータ収集は残り数例で終了予定となっている。

Reason

対象者のリクルートが難渋したものの、従来の研究計画通り6施設の往診クリニックで本調査を開始することができた。2023年度では改良されたePRO型ACPモデルの導入完了およびアンケート調査を一部を除き終了することができ、おおむね順調に進展している。

Strategy for Future Research Activity

ePRO型ACPモデルの有効性を検証するために、モデルの入力データおよびアンケート調査結果の解析に加え、研究結果の公表を進めていく。
