Outline of Research at the Start

“てんかん”は最も頻度の高い神経疾患の一つであり、根本的な病態の理解と治療法の開発が待たれている。これまで抗てんかん薬は、シナプス伝達や神経細胞の興奮を担うイオンチャネルを主な標的として開発されてきたが、これら薬剤だけでは治療困難な例も多い。本研究では、イオンチャネル以外のてんかん原因遺伝子(ADAM22等)の変異に着目し、その変異による分子病態を解明すると共に、新たなてんかん治療戦略を提案する。

Outline of Annual Research Achievements

神経分泌タンパク質LGI1の病的バリアントは、遺伝性側頭葉てんかんを引き起こす。これまでに、私共はLGI1の受容体としてADAM22を見出し、昨年度(2022年度)は、国際共同研究により、ADAM22のホモ接合型バリアントもしくは複合ヘテロ接合型バリアントを有する19名の常染色体潜性遺伝のてんかん脳症患者を見出し、「ADAM22てんかん性脳症」という新たな疾患分類を提唱した(Brain 2022)。そして、13種類のADAM22病的バリアントの分子病態を明らかにした。本年度(2023年度)はさらに、チェコ共和国のNoskova 博士等との共同研究にて、新たなADAM22の病的バリアント(ADAM22c.2714C>T; S905F)を見出した(Noskova L, Fukata Yら, Brain Commun 2023)。両アリルにADAM22 S905Fを有する児は、焦点てんかん、中程度の神経発達症を示した。私共はS905FがADAM22のC末端領域に位置し、足場タンパク質PSD-95との結合モチーフに相当することに着目し、両者の結合に及ぼす影響を検討した。その結果、ADAM22 S905Fは野生型ADAM22に比較して、PSD-95に対する結合能が約20%にまで低下していることを見出した。さらに、ADAM22 S905FはPSD-95ファミリーに属するPSD-93やSAP102に対しても同様に結合能が低下していることを見出した。興味深いことに、約5%のローマ系の人々はこのS905Fバリアントをヘテロ接合体として有することが示唆された。また、ミオキミア症状や、知的発達症、痙攣発作を呈する患者で報告されたLGI3バリアントについても解析を進め、その病態機構やLGI3の生理機能についても明らかにした(Miyazakiら,Cell Rep 2024)。

Strategy for Future Research Activity

1. さらなるADAM22ファミリーやLGI1ファミリーのバリアントを探索する。
2. ADAM22ファミリーやLGIファミリーの病的バリアントの性状解析を進める。特にLGIファミリーの分泌量やADAM22ファミリーの細胞膜発現量への影響を定量的に調べる。
